自体球结膜移植治疗翼状胬肉的并发症分析

目的自体球结膜移植治疗翼状胬肉复发率较低,疗效好。为减少手术的术中及术后的并发症,本文对39例自体球结膜移植治疗翼状胬肉进行总结报道。方法对2006年至2008年12月在我院就诊的翼状胬肉患者行自体球结膜移植,并对其疗效及并发症进行观察。结果 38例（占97.4%）眼术后2～5d结膜供区上皮化。30眼（占77%）术后随访超过12个月,复发2例（占5%）,平均随访14.6个月,其中1眼因操作失误使供体粘膜面朝外,后供体溶解,术后6个月复发;1眼术后12个月复发。并发症：1例因操作失误使供体粘膜面朝外,后供体溶解,胬肉复发;10例术中缝合时致巩膜面出血,血残留于供体结膜与巩膜之间,愈合后术面不够平整且术面结膜充血时间较长,结膜供区上皮化时间较长,无出血者愈合后术面平整且无充血;2例显微缝线拆除过早,致供体结膜与受体未完全愈合;1例取供体面积较大致创面不能完全缝合。结论自体球结膜移植治疗翼状胬肉复发率较低,疗效好。减少术中及术后并发症的发生,可提高手术疗效。

Autogenous Conjunctival Transplantation m the Treatment of Pterygium Complications

Objective Autologous conjunctival transplantation pterygium,low recurrence rate has good curative effect.In order to reduce the operation of preoperative and postoperative complications,this article 39 cases of autologous conjunctival transplantation in the treatment of pterygium were reported.Methods To December 2006～2008 with pterygial patients with autologous conjunctival transplantation,and its effect on observation and complications.Results 38（2） eyes 97.4 % of conjunctival epithelial 5d-area.30 eyes（77%） follow-up of more than 12 months,2 cases（5%）,average follow-up 1.46 months,including 1 eye for operating errors that donor mucosal surface outwards,dissolved,after six months after CLT relapse.On December 1 eye surgery.For example：1 complication that donor mucosal surface outwards,dissolved,recurrent pterygia after donor,10 cases of ZhongFeng intraoperative hemorrhage,scleral surface gives blood donor conjunctiva and sclera residue in between,healing art and art face face is longer,conjunctival congestion of conjunctival epithelial change for the area for a long time,no bleeding after operation and no congestion smooth surface,Two cases of micro stitch,dismantling early donor fails to complete healing of conjunctival and receptors,1 take donor area is approximately wound not completely suture.Conclusion Autologous conjunctival transplantation pterygium,low recurrence rate has good curative effect.Reduce preoperative and postoperative complications,can improve the curative effect.
