芬太尼复合丙泊酚与瑞芬太尼复合丙泊酚用于无痛人流术的麻醉效果观察

目的观察芬太尼复合丙泊酚与瑞芬太尼复合丙泊酚用于无痛人流术的麻醉效果和安全性。方法将160例行无痛人流术的早孕妇女随机分为两组,即芬太尼复合丙泊酚组（A组,n=80）和瑞芬太尼复合丙泊酚组（B组,n=80）。对两组患者的麻醉镇痛效果进行评级,观察并记录两组患者麻醉前、手术中、手术结束时的血压（BP）、心率（HR）、呼吸频率（RR）、血氧饱和度（SpO2）变化,以及丙泊酚用量、清醒时间、离院时间和术后腹痛情况。结果两组患者麻醉前、手术中、手术结束时的BP、HR、RR、SPO2均有变化,但组间比较无统计学意义（P〉0.05）;两组的术中镇痛效果均满意,组间比较差异无统计学意义（P〉0.05）;但术后镇痛效果比较A组优于B组（P〈0.01）;两组丙泊酚用量、术毕清醒及离院时间比较差异无统计学意义（P〉0.05）。结论芬太尼复合丙泊酚与瑞芬太尼复合丙泊酚应用于无痛人流术麻醉,两种方法均安全可靠,镇痛效果满意,但芬太尼复合丙泊酚麻醉的术后镇痛效果更好。

Anesthetic effect of fentanil combined with propofol and remifentanil combined with propofol for induced abortion

Objective To detect the efficacy and safety of anesthesia with fentanil combined with propofol and remifentanil combined with propofol for induced abortion.Methods 160 patients of early pregnancy with voluntarily induced abortion were randomly divided into two groups.Group A（80 cases）was anesthetized with fentanil combined with propofol,whereas group B（80 cases）was anesthetized with remifentanil combined with propofo1.The changes of blood pressure,heart rate,breath rate and oxygen saturation were observed during the anesthesia.The dose of propofol,the time for recovery and leaving hospital and the incidence of abdominal pain after operation were recorded.Results The effect of analgesia during operation was satisfactory in both of group A and in group B;but the effect of analgesia after operation in group A was better than that of in group B（P〈0.01）.The consumed dose of propofol and both the time of recovery and leaving hospital were no different in two groups.Conclusion Anesthesia with both fentanil combined with propofol and remifentanil combined with propofol for induced abortion were safe and reliable,but the effect of analgesia with fentanil combined with propofol after operation was better than that of remifentanil combined with propofol.
