多重人乳头瘤病毒感染与宫颈癌及癌前病变的相关研究

目的探讨高危型人乳头瘤病毒（human papillomavirus，HPV）的感染与宫颈癌及癌前病变的关系。方法对就诊的有宫颈疾患的妇女483例，采用HPV基因分型技术进行高危型DNA的检测，阴道镜下定位活检。病理学结果为确诊标准。结果高危型HPV检测阳性180例，阳性率37．23％，各种病理类型的宫颈病变中高危型HPV感染率分别为：宫颈鳞癌13．89％，宫颈腺癌12．56％，CINⅢ32．77％，CINⅡ56．3％，CINⅠ44．58％，宫颈扁平湿疣86．69％，慢性宫颈炎79．59％。单一型别感染率为38．89％多型感染中2种型别感染比例最多57．86％。从正常组织到宫颈上皮内瘤样病变（CIN）Ⅱ的不同病变中，多重感染的比例随病变级别上升逐渐增加，与正常者相比差异有显著性意义（P〈0．05）。在CINⅢ和宫颈癌中，多重感染的比例有所下降，但明显高于正常者。HPV联合宫颈薄层液基细胞学（TCT）检测与单纯TCT检测比较，两者中低度鳞状上皮内病变（HSIL）为4．17％和0．78％，癌为0．56％和0．20％，两者差异有显著性意义，P〈0．01。结论结合HPV检测可提高TCT检测的敏感性，多重HPV感染及型别与宫颈病变级别相关。
