系统性红斑狼疮患者血浆血小板膜颗粒蛋白的变化及临床意义

摘    要:目的:探讨系统性红斑狼疮(SLE)患者血浆血小板膜颗粒蛋白(GMP-140)的变化及其意义。方法:用ELISA法检测39例SLE患者和15名正常人血浆血小板GMP-140的含量,同时用全自动血细胞分析仪检测血常规及用魏氏法测定血沉(ESR)。结果:1.活动期SLE患者(n=27)患者血浆血小板GMP-140含量[(49.81±5.85)μg/L]明显高于缓解期SLE患者[(21.92±3.67)μg/L],(n=12)和正常对照组[(15.74±2.33)μg/L](n=15),P均<0.01。而缓解期SLE患者血浆血小板GMP-140的含量也高于正常对照组(P<0.05)。27例活动期患者中有20例进行了治疗前后的对比观察,治疗后SLE患者血浆血小板GMP-140较治疗前明显下降,但仍高于正常(P<0.05)。2.活动期SLE患者血红蛋白(Hb)、白细胞计数(WBC)、血小板计数(Bpc)均明显低于缓解期患者(P<0.01),而血沉(ESR)高于缓解期患者(P<0.01)。3.血浆血小板GMP-140的变化与ESR呈直线正相关(r=0.67,P<0.01),与血常规参数均无相关性。结论:SLE患者体内存在有血小板活化,SLE患者在活动期血浆血小扳GMP-140升高,治疗后明显下降,且与FSR呈直线正相关,说明GMP-140的变化与疾病活动性有关,可作为SLE疾病活动的指标之一;同时对部分临床表现的解释有一定的临床意义。

Abstract:Objective:To explore the significance of change in the quantity of plasma platelet α-Cranule Membrane protein 140 (GMP-140) in patients with SLE.Methods:The quantity of platelet GMP-140 in plasma was detected by ELISA in 39 cases of SLE and 15 normal controls.Hemoglobin(Hb),White blood cell (WBC) .Blood platelet count (Bpc) and Erythrocyte Sedimentation Rate (ESR)were measured,too.Results:The quantity of plasma platelet GMP-140[(49.81 ± 5.85]μg/L(n = 27) in active SLE was significantly higher than that in remissive SLE[ (21.92 ± 3.67)μg/L]( n = 12) and normal controls[ (15.74 ± 2.33)μg/L]( n =15)(P < 0.01 and 0.01) .There was significantly higher quantity of plasma platelet GMP-140 in remissive SLE than that of normal controls (P < 0.05) .The level of plasma platelet GMP-140 in 20 cases of active SLE was observed before and after treatment. After treatment, the plasma platelet GMP-140 obviously declined, but it was higher than that of normal controls yet( P < 0.05). The levels of Hb, WBC and Bpc in active SLE were significantly lower than that in remissive SLE, but the ESR was higher than that of remissive SLE.There was a correlation between the level of plasma pktelet GMP-140 and ESR( r = 0.67, P < 0.01) .Conclusion:There is activation of platelet in patients with active SLE. It is correlated with activity of the disease. It has the clinical significance that the change of plasma platelet GMP-140 may become one of index of the disease activity and can be used for explanation in some clinical symptons of SLE.
