经乳晕双孔双通道腔镜甲状腺手术与传统手术的对比研究

目的:探讨经乳晕双孔双通道腔镜甲状腺手术的可行性和疗效。方法:按病人意愿,前瞻性地将30例单侧甲状腺肿瘤病人配对分为两组,腔镜组和传统组各15例。对两组的手术时间、肿瘤大小、标本重量、术后疼痛评分、并发症、血清C反应蛋白变化水平、术后住院时间、术后美容效果等进行对比研究。结果:30例手术均获成功。两组间年龄、肿块直径、切除标本重量和术后住院时间,无统计学差异。腔镜组术后视觉模拟疼痛评分和血清C反应蛋白值与传统组间无统计学差异。腔镜组手术时间较传统组为长。术后3个月随访,腔镜组对美容效果满意15例,满意率100%;传统组满意12例,不满意3例,满意率80%。腔镜组有颈部感觉减退1例;传统组有颈部感觉减退和吞咽不适感各3例。结论:经乳晕双孔双通道腔镜甲状腺手术是一种安全可行、创伤不大、疼痛较轻、美容效果极佳的手术方式。

Endoscopic thyroidectomy via the bilateral-nipple approach by dual channels versus conventional open thyroidectomy: a prospective study
