| 摘 要： | 目的]探讨对高龄慢性病病人实施护士主导下的居家连续性照护的效果。方法]将宁波明楼社区100例高龄慢性病病人分为观察组和对照组,每组50例,两组病人均给予常规随访,对观察组病人实施护士主导下的居家连续性照护,评价两组病人干预后治疗依从性、生活质量变化及满意度情况。结果]两组干预后半年,观察组的治疗依从性、生活质量及满意度状况均优于对照组(P0.05)。结论]对高龄老年慢性病病人实施连续性照护模式,能提高高龄病人治疗依从性和生活质量,增加社区护理的满意度。

Application effect of nurse led continuous care model in elderly patients with chronic diseases |

| Abstract: | Objective:To probe into the effects of nurse led continuous care model in elderly patients with chro-nic diseases.Methods:A total of 100 elderly patients with chronic diseases were randomly divided into interven-tion group(n=50)and control group(n=50).The intervention group received the continuous care model.The control group received usual community health service.Then compared changes of quality of life and treatment compliance,and satisfaction degree of the two groups patients.Results:After 6 months’intervention,changes of quality of life and treatment compliance,and satisfaction degree of the intervention group patients were better than that of the control group(P<0.05).Conclusion:The nurse led continuous care for elderly patients with chronic disease could help elderly patients to improve treatment compliance and quality of life,and to increase their satisfaction degree to community care. |
