老年高血压患者术前应用盐酸戊乙奎醚对术后早期认知功能改变的影响

目的观察术前使用盐酸戊乙奎醚对高血压老年患者认知功能障碍发生率的影响。方法选择行择期手术的高血压老年患者71例,随机分为戊乙奎醚组（40例）和对照组（31例）,戊乙奎醚组在麻醉诱导前10min静脉注射盐酸戊乙奎醚0.5mg,对照组则给予等容积生理盐水。分别在术前及术后第7天对所有患者采用简易智能量表（MMSE）进行神经心理学测试。结果两组患者围手术期血压水平差异无统计学意义（P〉0.05）,术后7dMMSE评分≤23分的患者戊乙奎醚组和对照组分别有6例（14.3%）和5例（13.2%）,术后认知功能障碍发生率组间比较差异无统计学意义（P〉0.05）。结论麻醉诱导前静脉注射盐酸戊乙奎醚0.5mg对高血压老年患者术后的认知功能发生率无明显影响。

Effect of using penehyclidine hydrochlorid prior to operation on the cognitive function in the elder patients with hypertension post surgery

Objective To investigate the impact of penehyclidine hydrochlorid prior to operation on the incidence of postoperation cognitive dysfunction（POCD） in the aged patients with hyperstension.Methods 71 elderly patients were randomly divided into the penehyclidine hydrochlorid（Ph） group and control group.In Ph group,the patients were given 0.5mg penehyclidine hydrochloride intravenously prior to induction,while the same-volume saline was given in control group.A neuro-psyehologieal test using Mini-Mental state examination MMSE were carried out on all the patients prior to surgery and 7 days post operation.Results No statistical significance was shown in blood pressure between two groups peri-surgery.Among the patients with the MMSE score under 23,six cases belonged to the Ph group（14.3%） and five cases were in the control group（13.2%）.The occurrence of POCD showed no statistical difference between two groups.Conclusion The administration of penehyclidine hydrochlorid preoperatively for the elderly patients with hypertension has no the impact on the incidence of POCD.
