复合静脉麻醉实施无痛人工流产800例临床观察

摘    要:目的 探讨复合静脉麻醉在无痛人工流产术中的临床疗效及副反应。方法 随机选择 80 0例早孕自愿要求行无痛人工流产术者 ,先静脉推注芬太尼和阿托品的混合液 1.5ml,再接着静脉注射异丙酚原液至病人入睡 ,术中视病人需要和病人反应可追加麻醉药物 ,并与 2 0 0例宫旁阻滞麻醉者 (局麻组 )相比较。结果 复合静脉麻醉者在人工流产术中镇痛效果确切 ,其有效率达 99.5 % ,0 .4 %病人术中出现短暂性呼吸停止 ,81%的宫颈口完全松弛 ,其人工流产综合征发生率和不全流产率低于局麻组 ,两组阴道出血量无明显差异性 ,复合麻醉组术后 5min即可清醒 ,术后 1月随访无异常。结论 复合静脉麻醉是一种安全简单值得临床推广的无痛人工流产方法。

Abstract:Objective To discuss the clinical effect of compound intravenous anesthesia on induced abortion and its adverse reaction.Methods 800 cases of early pregnancy that asked for painless inducad abortion were enrolled in at free will at random.15 ml of mixture of Fentanyl and atropine was given to them through intravenous injection,and then propofol was given so that they fell asleep.Additional anesthesia was administered in the course according to the cases'responses to the operations.They were compared with another 200 case of early pregnancy that accepted painless induced abortion through local block anesthesia of uterus.Results Compound intravenous anesthesia was clarly effective on easing pain with an efficiency of 99.5%.81% of the cases reach complete cervical relax.The incidence of induced abortion syndrome or incomplete abortion was lower than that of local block anesthesia.Only in 0.4% of the cases appeared transient respiratory trouble.Conclusion It is deemed that compound intravenous anesthesia be safe,simple,effective,and worthy of application to painless induced abortion in clinic.
