腹腔镜直肠癌根治术93例临床体会

目的:探讨腹腔镜直肠癌根治术的临床优势、注意事项及手术技巧。方法:回顾分析93例腹腔镜直肠癌根治术的临床资料,总结腹腔镜直肠癌根治术的手术优势、术中注意事项及手术技巧。结果:1例中转姑息性造口术,92例顺利完成手术,其中23例行Miles术,69例行Dixon术;手术时间90¹80 min,平均(148.0±32.2)min;术中出血量10180 min,平均(148.0±32.2)min;术中出血量10¹00 ml,平均(62.5±23.1)ml;肠道功能恢复时间2100 ml,平均(62.5±23.1)ml;肠道功能恢复时间2³ d,术后住院73 d,术后住院7¹1 d,平均(8.6±1.2)d;无围手术期死亡、术后出血、尿潴留、肠梗阻等并发症发生。结论:与传统开腹手术相比,腹腔镜直肠癌根治术具有明显优势,掌握手术操作技巧与注意事项后可更顺利地完成手术。

Clinical experience of 93 cases laparoscopic radical resection of rectal carcinoma

Objective：To study the clinical advantages, precautions and surgical techniques of laparoscopic radical resection of rectal carcinoma. Methods ：The clinical data of 93 cases of laparoscopic radical resection of rectal carcinoma were retrospectively ana- lyzed. The advantages,intraoperative precautions and surgical techniques of laparoscopic radical resection of rectal carcinoma were sum- marized. Results：Except 1 case was converted to palliative ostomy,92 cases were successfully operated. Among the 92 cases,there were 23 cases of Miles operation and 69 cases of Dixon operation. The operation time was 90-180 min, the mean was （ 148.0 ± 32.2 ） min. The amount of intraoperative hemorrhage was 10 to 100 ml,with the average of （62.5 ± 23.1 ） ml. The postoperative recovery time of intestinal function was 2 to 3 days. The hospital stay after surgery was 7 to 11 d mean, （ 8.6± 1.2 ） d ]. No postoperative bleeding, perioperative mortality ,urinary retention, intestinal obstruction or other complications occurred. Conclusions： Laparoscopic radical re- section of rectal carcinoma has significant advantages compared with traditional open surgery, mastering the intraoperative skills and pre- cautions makes the operation smoother and easier.
