CyclinD1和CDX2蛋白在胃癌组织中的表达及临床意义

摘    要:目的:探讨细胞周期素D1(CyclinD1)和尾型同源转录因子-2(CDX2)蛋白的表达与胃癌(GC)临床病理特征及患者生存期的关系。方法:应用免疫组织化学技术检测120例GC、60例肠上皮化生(1M)、60例异型增生(GED)$020例正常胃黏膜(NGM)组织中CyclinD1和CDX2蛋白的表达,并结合GC的病理学行为和临床随访资料进行分析。结果:CyclinD1在GC组织中表达阳性率为61.7%,显著高于GED、IM和NGM组织(35.0%、167%、O),差异有统计学意义(P〈O.05),GED组织高于IM和NGM组织,差异亦有统计学意义(P〈0.05)。CDX2在GC、GED、IM组织中表达阳性率分别为56.7%、68.3%和917%,显著高于NGM组织(O),差异有统计学意义(P〈005);IM组织高于GC和GED组织,差异亦有统计学意义(P〈O05);GC组织与GED组织之间差异无统计学意义(P〉O05)。CyclinD1和CDX2表达与Gc分化程度、浆膜浸润、淋巴结转移和患者生存期相关(P〈0.05)。CyclinD1和CDX2表达呈显著负相关(r=-0.34,P〈O,05)。结论:CyclinD1和CDX2均参与了GC的发生发展,均可作为辅助GC诊断、预测生物学行为和评估预后的参考指标。

Abstract:Objective: To investigate the expression of adhesion molecules Cyclin D1 and CDX2 protein in human gastric carcinoma in the aged, and their relationship with the clinicopathologic charac- teristics and prognosis. Methods: Immunohistochemical staining was used to detect the expression of Cyclin D1 and CDX2 protein in 120 cases of gastric carcinoma (GC), 60 cases of intestinal metaplasia (IM), 60 cases of gastric epithelial dysplasia(GED) and 20 cases of normal gastric mucosa(NGM). The study was also combined with analysis of the clinicopathotogic characteristics and clinical follow-up data of GC. Results.The positive rates of Cyclin D1 in GC (61.7%) patients was higher than that in GED, IM and NGM (35.0%, 16.7%, 0, P〈 0.05) respectively; The positive rates of Cyclin D1 in GED patients was higher than that in IM and NGMCP〈0.05);The positive rates of CDX2 in GC, GED and IM(56.7%, 68.3%, 91.7%) were higher than that in NGM(P〈 0.05). The positive rates of CDX2 in IM were higher than that in GC and GED (P 〈 0.05). The expression of Cyclin D1 and CDX2 protein was positively cor- related with tumor grading, serosa infiltration, lymph node metastasis and survival of GC (P〈 0.05). The expression of Cyclin D1 was negatively correlated with that of CDX2 in GC (r=-0.34, P〈0.05). Consion: Cyclin D1 and CDX2 are both involved in the occurrence and development of GC. Cyclin D1 and CDX2 both might be reference indexes to assist diagnosis, predict and judge prognosis in GC.
