同时多层采集技术在乳腺MR高清扩散峰度成像中的可行性研究

摘    要:目的:探讨同时多层采集技术在乳腺高清扩散峰度成像(RESOLVE DKI)中的应用价值。方法:回顾性分析行乳腺MRI检查并经病理学证实的46例女性患者(48个病变,其中恶性病变41个,良性病变7个)的临床及影像资料,分别采集常规RESOLVE DKI和采用同时多层采集技术RESOLVE DKI序列(SMS RESOLVE DKI)的数据。DKI相关的定量参数平均峰度值(MK)、平均扩散系数(MD)和表观扩散系数(ADC)值通过第三方软件(DKI_tool_0.2.2)进行测量。同时对所有图像的锐利度、几何变形、图像对比度、病灶可见度、解剖细节的显示及总体图像质量进行评价。统计分析常规RESOLVE DKI与SMS RESOLVE DKI在定量参数和图像评估上的差异及对良恶性病变的鉴别诊断效能。结果:与常规RESOLVE DKI相比,SMS RESOLVE DKI的扫描时间缩短48.7%(分别为4分27秒和2分17秒)。常规RESOLVE DKI与SMS RESOLVE DKI之间各定量参数均呈良好的线性相关性(MK、MD、ADC的相关系数分别为0.965、0.950、0.984,P值均<0.001);RESOLVE DKI与SMS RESOLVE DKI定量参数对乳腺良恶性病变的鉴别诊断效能差异无统计学意义。传统RESOLVE DKI与SMS RESOLVE DKI之间的图像锐利度、几何变形、图像对比度、病灶可见度、解剖细节显示以及总体图像质量差异均无统计学意义。结论:与传统的RESOLVE DKI技术相比,SMS RESOLVE DKI可以显著减少图像采集时间并产生高度一致的定量参数、相似的诊断效能和可比的图像质量,有助于扩大高清扩散技术在乳腺成像中的临床应用范围。

Feasibility study of readout-segmented diffuse kurtosis imaging with simultaneous multislice in breast MRI

Abstract:Objective:To explore the application of readout-segmented diffuse kurtosis imaging(RESOLVE DKI)with simultaneous multislice in breast MRI.Methods:Forty-six female patients underwent breast MRI examinations from March to September 2019 were retrospectively included in this study.A total of 48 breast lesions(41 malignant and 7 benign lesions)were confirmed by pathological examinations after surgery or needle biopsy.Both conventional RESOLVE DKI and simultaneous multislice(SMS)RESOLVE DKI sequences were performed for each patient before the surgery or biopsy procedure.The DKI-derived quantitative parameters including mean kurtosis(MK),mean diffusion(MD)and apparent diffusion coefficient(ADC)values were measured by an open-source software(DKI_tool_0.2.2).The sharpness,geometric distortion,image contrast,lesion conspicuity,visualization of anatomical details,and overall imaging quality of all images were evaluated by a blinded read.Statistical analysis of the differences and correlations between conventional RESOLVE DKI and SMS RESOLVE DKI results in quantitative and qualitative parameters as well as diagnostic efficiency were performed.Results:Compared with conventional RESOLVE DKI,the acquisition time of SMS RESOLVE DKI was significantly reduced by 48.7%(4′27″vs 2′17″).The DKI-derived parameters between conventional RESOLVE DKI and SMS RESOLVE DKI showed good linear correlation(the correlation coefficients of MK,MD,and ADC were 0.965,0.950,and 0.984 respectively,all P<0.001).Image quality of SMS RESOLVE DKI was comparable to conventional RESOLVE DKI.There was no statistically significant difference between RESOLVE DKI and SMS RESOLVE DKI neither in the quantitative parameters nor in the differential diagnosis of benign and malignant breast lesions.There were also no statistically significant differences between the quantitative parameters of RESOLVE DKI and SMS RESOLVE DKI in the sharpness,geometric distortion,image contrast,lesion conspicuity,visualization of anatomical details,and overall imaging quality of all images either.Conclusion:Compared with conventional RESOLVE DKI,SMS RESOLVE DKI can significantly reduce the acquisition time and produce highly consistent quantitative parameters,similar diagnostic accuracy and comparable image quality,which may help to expand its clinical application in breast imaging.
