呼和浩特市老年干部血25(OH)D及与PTH、骨密度、骨折风险的关系

摘    要:目的 通过调查呼和浩特市老干部的维生素D营养状况,和对骨密度及骨折风险的影响,为进一步防治骨质疏松及维生素D缺乏引起的各种疾病提供理论基础。方法 随机选取2012年11月~2013年9月在内蒙古自治区人民医院干部保健所就诊、体检的年龄60岁以上的干部268例,最终入选235例。对入选对象进行一般情况调查,并抽血测定25(OH)D、PTH等生化指标,同时进行骨密度测定。对所有数据进行整理,用IBM SPSS统计软件包进行统计分析。结果 235名研究对象中,血25(OH)D浓度为13.51±8.59ng/mL。PTH浓度为45.99±20.02pg/mL。骨密度:股骨颈为0.690±0.144 g/cm2,总髋部为0.818±0.158 g/cm2,腰椎1-4为0.978±0.207 g/cm2。10年主要骨质疏松骨折概率为3.29±2.66%, 10年髋部骨折概率1.36±1.59%。总25(OH)D水平与10年主要骨质疏松骨折概率、10年髋部骨折概率均负相关。结论 呼和浩特市老年干部维生素D缺乏和不足十分严重,骨质疏松患病率较高,应引起足够重视。

Abstract:Objectives To study the vitamin D status and its effect on BMD and fracture risk in the elderly cadres in Hohhot, in order to provide theoretical reference for further prevention and treatment of various diseases caused by osteoporosis and vitamin D deficiency. Methods A total of 268 over 60 years old cadres who received treatment or physical examination in Cadre Health Centre of People''s Hospital of Inner Mongolia Autonomous from November 2012 to September 2013 were randomly selected, and 235 of them were finally selected in this study. Questionnaire survey was performed in the selected subjects to understand their general condition. 25(OH)D, PTH, and biochemical markers were examined. Bone mineral density (BMD) was measured. All of the data obtained were statistically analyzed with IBM SPSS statistical package. Results In the 235 subjects, serum 25(OH)D concentration was 13.51±8.59 ng/ml, PTH concentration was 45.99±20.02 pg/ml, and BMD was 0.690±0.144 g/cm2 in the femoral neck, 0.818±0.158 g/cm2 in the total hip, and 0.978±0.207 g/cm2 in the lumbar vertebrae 1-4. The 10-year major osteoporotic fracture probability was 3.29±2.66%. The 10-year hip fracture probability was 1.36±1.59%. The total 25(OH)D level was negatively correlated with 10-year major osteoporotic fracture probability and 10-year hip fracture probability. Conclusion Vitamin D deficiency and insufficiency in elderly cadres in Hohhot are very serious, and the prevalence of osteoporosis is high, which should be paid close attention.
