替加环素、米诺环素、多黏菌素B对碳青霉烯类敏感性降低和不敏感肺炎克雷伯菌的体外抗菌活性

摘    要:目的评价替加环素、米诺环素、多黏菌素B等对碳青霉烯类抗生素敏感性降低肺炎克雷伯菌(CDSKp)和碳青霉烯不敏感肺炎克雷伯菌(CNSKp)体外抗菌活性,为临床治疗该类菌感染提供依据。方法采用琼脂稀释法检测替加环素、米诺环素、多黏菌素B等抗生素对临床分离51株CDSKp和101株CNSKp的最低抑菌浓度(MIC)。结果多黏菌素B对CDSKp和CNSKp体外抗菌活性最高,细菌对其均100%敏感,MIC50和MIC90均为0.125mg/L和0.25mg/L;替加环素MIC50均为1mg/L,MIC90均为2mg/L;米诺环素MIC50均为4mg/L,MIC90分别为4mg/L和8mg/L。结论多黏菌素B、替加环素和米诺环素对CDSKp和CNSKp具有较强的体外抗菌活性。

Abstract:Objective To investigate the effects of tigecycline, minocycline and polymyxin B against Klebsiellapneumoniae with decreased or no susceptibility to carbapenems in vitro. Methods Clinical strains of Klebsiella pneumoniae were collected in Yiwu Central Hospital during the period of 2008 to 2014, including 51 strains with decreased susceptibility to carbapenems and 101 non-susceptible strains. Agar dilution method was used to determine the minimum inhibitory concentrations (MICs) of tigecycline, minocycline and polymyxin B against Klebsiella pneumonia. The data were analyzed by Windows software SPSS 16.0. Results Polymyxin B showed the highest activity against K. pneumonia strains with decreased or no susceptibility to carbapenems, and all the strains were susceptible to polymyxin B in vitro with a MIC 50 and MIC90 value of 0.125mg/L and 0.25mg/L, respectively. The MIC50 and MIC90 values for tigecycline were 1mg/L and 2mg/L, respectively; those for minocycline were 4mg/L and 8mg/L, respectively. Conclusion Tigecycline, minocycline and polymyxin B have relatively higher antibacterial activity against K. pneumonia strains with decreased or no susceptibility to carbapenems.
