鼓室内地塞米松注射治疗糖尿病患者突发性聋临床研究

目的 观察鼓室内地塞米松注射治疗糖尿病人突聋的临床效果及安全性。方法 将55例患有糖尿病的突聋患者随机分为治疗组(35例)和对照组(20例),治疗组在扩血管等一般性治疗的基础上再予鼓室内注射地塞米松, 将5g／L地塞米松0.5ml注入鼓室,隔日1次,１０天为1疗程；对照组只予扩血管等一般性治疗。一个疗程结束时重复测试纯音听阈,按照2005年济南会议确定的疗效分级标准评定疗效,比较两组有效率。并观察治疗组和对照组的空腹血糖变化。结果 治疗组中24例有效,11例无效,有效率为68.57%;对照组中5有效例,无效15例,有效率为25%;治疗组血糖异常发生记录到2次(0.06%),对照组血糖异常发生记录到1次(0.05%)。治疗组疗效明显优于对照组,两组的有效率差异有显著性(P＜0.01)；治疗组和对照组的血糖异常发生率差异无显著性(P＞0.05)。 结论 鼓室内地塞米松注射治疗糖尿病人突发性聋是有效的、安全的,可考虑作为有激素禁忌证的突发性聋患者的治疗方法。

Clinical research of drum chamber dexamethasone injection for sudden hearing loss in patients with diabetes

Objective To investigate efficacy and safety of drum chamber dexamethasone injection for hearing loss in patients with diabetes. Method 55 patients with diabetes and sudden hearing loss were randomly divided into treatment group (35 cases) and control group (20 cases), The treatment group was given vasodilators and injecting to drum chamber of dexamethasone, 5g/ L dexamethasone 0.5ml injected into the attic, every other day, 10 days for a course of treatment; control group only to vasodilator treatment. At the end of a course, testing pure tone audiometry to assess the clinical efficacy grading standards determined in accordance with the 2005 Jinan meeting, the effective rate were compared between the two groups. And observe the changes in the treatment group and the control group of fasting blood glucose. Result In the treatment group, 24 cases effective, 11 cases were ineffective, the effective rate of 68.57%; in the control group, 5 cases effective, 15 cases ineffective, the effective rate of 25%; treatment group abnormal glucose records to two times (0.06%) in the control group abnormal glucose record to 1 (0.05%). The treatment group was significantly better than the control group, the efficiency difference between the two groups was significant (P <0.01); the abnormal glucose rate between treatment group and the control group was no significant difference (P> 0.05).Conclusion Drum chamber dexamethasone injection for sudden hearing loss with diabetes is safe, effective, Can be considered as a treatment for hormone contraindications in patients with sudden hearing loss.
