闭合性胰腺损伤的诊断和治疗

目的探讨腹部闭合性损伤中胰腺损伤的诊断和治疗。方法分析我院近10年来收治的69例确诊为胰腺损伤患者的临床资料。结果术前B超检查阳性率为32.1%,CT检查阳性率为22.5%,腹腔穿刺液淀粉酶(Ams)升高占54.1%,血Ams升高占59.4%,尿Ams升高占63.8%,69例保守治疗6例,手术治疗63例,死亡11例,病死率为17.5%。结论腹部闭合性损伤中胰腺损伤术前诊断较困难,外科医生要提高对胰腺损伤的警惕性,术中认真探查,视胰腺损伤的程度及合并伤选择合理的术式,少数患者可以保守治疗。

Diagnosis and Treatment of Blunt Pancreatic Injury(BPI)

Objective To investigate the diagnoses and treatment of blunt pancreatic injury(BPI).Methods A retrospective analysis was made on the clinical date of 69 cases treated in our hospital during 10 years.Results The positive diagnosis rate was 32.1% for BUS examination preoperatively,but 22.5% for CT scannings.The Ams in the ascite,blood,urine were examined,they were increased.In the 69 cases,6 cases received nooperative treatment,63 cases received operation,11 cases died,the mortality was 17.5%.Conclusion It is difficult to diagnose BPI before the operation,so the surgeon must enhance the vigilance and explore carefully.Appropriate methods should be chosen according to the injury degree,a few cases can receive conservative treatment.
