经阴道超声评价输卵管妊娠早期血流动力学变化

目的:探讨输卵管妊娠早期子宫动脉及其输卵管支血流动力学的变化。方法:临床上筛选出70例妊娠试验阳性,但子宫内外均无特异性表现的患者,应用经阴道彩色多普勒超声检测双侧子宫动脉及其输卵管支,测量并比较两侧子宫动脉及其输卵管支阻力指数的差异。结果:70例患者中26例最终诊断为输卵管妊娠,42例为宫内早孕,2例为宫内早孕流产。输卵管妊娠组中18例患者患侧子宫动脉阻力指数较健侧低,2例患侧子宫动脉阻力指数较健侧高,6例两侧相等,但差异无统计学意义(P>0.05);17例患者患侧输卵管动脉阻力指数较对侧低,2例较健侧高,5例两侧相等,2例只探及单侧输卵管动脉,其差异有统计学意义(P<0.05)。而宫内妊娠组双侧子宫动脉及其输卵管支阻力指数差异均无统计学意义(P>0.05)。结论:本研究表明,经阴道彩色多普勒超声检测并比较双侧子宫动脉的差异难以早期预测输卵管妊娠,但输卵管动脉血流参数的差异性能够为早期预测输卵管妊娠提供一定的帮助。

Transvaginal Color Doppler Investigation of the Hemodynamics in Early Tubal Pregnancy

Objective: To evaluate the value of hemodynamic changes of early tubal pregnancy(TP) in dignosing ectopic pregnancy.Methods: Seventy patients with positive pregnancy test and with atypical ultrasonograph results were involved,and the bilateral uterine atery and tubal artery were detected by transvaginal color Doppler(TVCD).The resistant index(RI) was measured and compared between each side.Results: Twenty-six patients were dignosed with TP,forty-two cases with uterine pregnancy,and two cases with abortion.The RI of uterine artery between the left and right had no significant difference in all cases(P>0.05).The RI of TP side was lower than the other side in 17 patients in the TP group,2 were higher than the other side and 5 were the same.The difference of the RI of tubal artery between sides has no statistical significance(P>0.05).Conclusion: The difference of the hemodynamic parameters of bilateral tubal artery can provide additional information for the dignosis of tubal pregnancy.
