留置静脉导管引流心包积液并发症的探讨

目的 留置静脉导管引流是治疗心包积液和心包填塞的有效方法，本旨在探讨心包穿刺引流的并发症。方法 本对36例中6例发生了心包穿刺引流并发症的住院病人进行分析。结果 在6例并发症中，有2例因导管前端刺激发生神经介导性晕厥，1例因引流过速而猝死，1例发生急性右心室扩张并死亡，1例发生心包积液经穿刺孔漏入左侧胸腔，1例发生心包积液经穿刺孔渗漏入皮下。结论 应充分认识留置引流所产生的血液动力学改变，对危重病人应行床边血液动力学监测。同时应注意引流导管本身对患可能造成的损伤。

Clinical study of complications caused by retention laying vena duct drainage in treating pericardial effusion

Objectives Retention laying vena duct drainage is an effective method to treat pericardial effusion and tamponade. We try to study the pericardiocentesis complications. Methods We analysed the clinical feature of 6 patients who had complications in 36 patients treating with pericardiocentesis. Results In 6 patients, 2 patients happened nervous syucope for stimunation of front duct part; 1 patient was sudden death for speedy drainage; 1 patient had acute right ventricle dilation and died; the fluid of pericardial effusion was leaked into left pleura and subcutis through pericardiocentesis hole in 2 patients respectively. Conclusions Retention laying subclavicular vena duct drainage will affect blood circulation motion, bedside monitors should be given to severe patients, the probable injury of patients caused by drainage duct itself should be supervised carefully.
