冬眠瘤的临床病理观察

目的探讨冬眠瘤的临床病理特点、诊断和鉴别诊断,提高对此瘤的认识。方法对3例冬眠瘤进行光镜观察,应用S-100蛋白、CD34等进行免疫组织化学染色分析其本质,并复习文献。结果组织学特点：肿瘤呈分叶状结构,为纤维血管分割,肿瘤细胞为圆形、多角形,胞质丰富,部分胞质内见嗜酸性颗粒及许多小脂滴,细胞核小,居中。免疫组织化学显示S-100蛋白阳性、CD34阴性。结论冬眠瘤病理特征显著,应注意与粘液样脂肪肉瘤、成年型横纹肌瘤、黄色瘤、颗粒细胞瘤鉴别。

Clinicopathological observation of Hibernoma

Objective To explore the clinicopathological characteristics,diagnosis,differential diagnosis of hibernoma so as to expand the knowledge of this tumor.Methods Three cases of hibernoma were studied by light microscopy.Its features were analyzed by immunohistochemistry with S100,CD34 etc.,and related literatures were reviewed.Results Histologically,the tumor was composed of numerous lobules separated by well vascularised connective tissue.These lobules were solid nests of tumor cells.The tumor cells were round or polygonal in shape.Most tumor cells were large number of multivacuolated brown fat cells with aboudant granular or eosinophilic cytoplasm and had a centrally located small and round nucleus.Immunohistochemical study demonstrated hibernoma cells were positive for S-100 protein and negative for CD34.Conclusions Hibernoma has distinctive histopathologic features.Its differential diagnosis involves myxoid liposarcoma,adult rhabdomyoma,xanthoma,and granular cell tumor.
