多模式低阿片类镇痛方案对改善患者术后睡眠质量的研究

目的 探讨多模式低阿片类镇痛方案对改善患者术后睡眠质量的作用.方法 选取诊断为胆总管结石的择期手术患者40例,根据术后应用镇痛药的种类分为多模式组(多模式阵痛方案)和对照组(常规镇痛方案),每组20例,观察患者术后睡眠时间及匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI).结果 2组患者手术当日的睡眠时间为3.2h,在随后的3d内上升到5h以上,术后7d的PSQI均在10分以上.2组患者术后睡眠时间、PSQI,多模式组优于对照组,差异有统计学意义(P＜0.05).结论 有效控制疼痛对于改善睡眠质量具有重要作用.镇痛药有助于改善术后睡眠障碍,促进患者术后恢复.

Effects of multi-modal low-opioid analgesia on postoperative sleep disturbance of patients undergoing common duct exploration

Objective To investigate the effect of multi-modal low-opioid analgesia regimen on improving postoperative sleep quality.Methods Forty patients undergoing elective surgery for common bile duct stones were selected.According to the type of postoperative analgesics,the patients were randomly divided into multimodal group (multimodal pain relief program) and control group (conventional analgesia scheme) with 20 patients in each group.Postoperative sleep time and Pittsburgh Sleep Quality Index (PSQI) were observed and compared.Results Patients of both groups had only 3.2 hours of sleep on the day of surgery,but rose to more than 5 hours in the following 3 days.PSQI at 7 days postoperatively was more than 10 points in both groups.The postoperative sleep time and PSQI of the two groups were better in the multimodal group than in the control group (P ＜ 0.05).Conclusion Although pain is not a major cause of postoperative sleep disturbances,effective pain control plays an important role in improving sleep quality.Multi-modal analgesia can help improve postoperative sleep disorders and promote postoperative recovery.
