穴位注射对慢性肾功能衰竭患者促红素抵抗的影响

目的:比较穴位注射和皮下注射药物对慢性肾功能衰竭患者促红素抵抗的影响。方法:将38例患者随机分为穴位注射组和皮下注射组,各19例。皮下注射组予皮下注射人类重组促红细胞生成素(rHuEpo),每周3次,治疗2个月;穴位注射组于肾俞、足三里穴注入rHuEpo,每次注射一个穴位,左右交替,每周3次,治疗2个月;同时选取19名健康人作为正常对照组,观察C反应蛋白(CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、血肌酐(Scr)、血尿素氮(BUN)、血红蛋白(Hb)、红细胞压积(Hct)、血清铁蛋白(SF)。结果:两组患者治疗前CRP、IL-6、TNF-α均高于正常对照组(均P0.01);治疗2个月后,两组CRP、IL-6、TNF-α、Scr、BUN明显降低,Hb、Hct、SF明显升高,与治疗前相比,差异有统计学意义(P0.05,P0.01);两组治疗后比较,穴位注射组改善程度优于皮下注射组(P0.05,P0.01)。结论:足三里、肾俞穴位注射rHuEpo可增加Hb、Hct、SF,降低BUN、Scr及炎性因子CRP、IL-6、TNF-α水平,疗效优于皮下注射。穴位注射通过减轻机体微炎性反应状态从而改善促红细胞生成素(Epo)抵抗,增强Epo疗效。

Effect of acupoint injection on erythropoietin resistance in patients with chronic renal failure

Abstract: Objective: To observe the influence Of Point Injection therapy in resistance of erythropoietin using to treat micro-inflammation state in Chronic Renal Failure. Methods: 38 cases of patients with chronic renal failure were divided into two groups randomly.19 cases of the therapy group were treated with rHuEpo point injection while the control group with rHuEpo point subcutaneous injection. Results: Concentration of CRP,TNF-α,IL-6 in all patients before therapy were significantly increased than normal group(P<0.01).After two months of therapy,Concentration of CRP,TNF-α,IL-6,rHuEPO/Hct,Scr,BUN in therapy group were decreased, while Hb,Hct,SF were increased, contrasted to control group, there was a significant difference(P<0.05). Conclusion: Point Injection therapy can improve the micro-inflammatory state in chronic renal failure ,and decrease resistance of erythropoietin.
