The level of plasma aldosterone in patients with chronic heart failure before atrial fibrillation occurring.

Objective To investigate changes of plasma aldosterone for incidence of atrial fibrillation （AF） in chronic systolic heart failure （CSHF） patients without AF. Methods Two hundred forty and three consecutive patients with CSHF were prospectively followed after biochemical measurement, echocardiography, and 24-hour Hoher monitoring at study entry. Results After follow up of 28 ± 5 months, group I included patients （n = 177） in whom had sinus rhythm, and group II included those （n = 47） who developed AF. Patients in group II had a significantly larger left atrial diameter（ LAD）, hypertension, age and level of plasma aldosterone than in group Ⅰ. While the left ventricular ejection fraction（LVEF） were higher in group Ⅰ than in group Ⅱ. Multivariate logistic regression analysis identified age, LAD, LVEF and level of plasma aldosterone as independent predictors of incidence of AF. Conclusions ： CSHF patients with developed AF had higher level of plasma aldosterone.
