3D双反转恢复序列在非颞叶致癫灶定位中的价值分析

摘    要:目的 对比分析非颞叶致癫灶在颅脑3D双反转恢复序列和FLAIR序列上的病灶信号对比度,探讨3D双反转恢复序列在非颞叶致癫灶定位中的价值.方法 选择在我院经临床诊断为癫痫患者52例,行颅脑常规扫描和3D双反转恢复序列扫描,发现非颞叶致癫灶患者13例.13例患者中,在3D DIR和FLAIR序列上以相同的体素选择病灶和邻近正常脑组织为感兴趣区,获得感兴趣区信号强度的平均值,病灶和邻近正常脑组织的信号强度分别以SI1和SI2表示,分别计算3D DIR和FLAIR序列上病灶的对比度.统计学分析采用SPSS19.0软件进行统计分析,对在3D DIR和FLAIR序列上致癫灶的对比度采用配对资料的符号秩和检验.结果 Z=-3.180,P=0.001<0.05,两个序列上致癫灶的对比度存在统计学差异.结论 3D DIR序列能够较FLAIR序列更明确的检出癫灶患者脑内的非颞叶异常信号,提高非颞叶致癫灶的检出,更准确定位非颞叶癫痫病灶.

Abstract:Objective To compare the contrast ratio (CR) of non-temporal lobe epileptogenic lesions on 3D double inver- sion recovery (DIR) and FLAIR images, and to explore the value of 3D DIR sequence in localizing non-temporal lobe epi- leptogenic lesion. Methods 52 patients clinically diagnosed with epilepsy in our hospital were included in this study. All patients underwent cerebral conventional MR imaging and 3D DIR imaging. Non-temporal lobe epileptogenic lesions were detected in 13 patients. The lesion and adjacent normal cerebral tissue with same voxels were selected as regions of interest in which the signal intensities were obtained. Contrast ratio (CR) of epileptogenic lesions on FLAIR and 3D DIR images were calculated. Whether there was significant difference of the CR on two sequences was determined by SPSS 19.0. Wil- oxon test for pared data was used for statistics. Results There was significant difference between the CR of epileptogenic lesions on FLAIR and 3D DIR images ( Z =-3. 180, P =0. 001〈0.05). Conclusion 3D DIR sequence can obviously de- tect the abnormal signal of the non-temporal lobe epileptogenic lesions, and accurately localize non-temporal lobe epilepto- genic lesions.
