新生儿骨碱性磷酸酶测定与先天性佝偻病相关因素分析

目的 探讨骨碱性磷酸酶筛查先天性佝偻病的价值，了解新生儿先天性佝偻病的发病率及其相关因素。方法 用全血干化学免疫浓缩法对630例新生儿进行了骨碱性磷酸酶活性的测定。分析BALP活性水平及其影响因素的相关性。同时进行血清钙、磷、碱性磷酸酶测定的对比。结果 受检630例，BALP≤200U／L430例，～250U／L82例、～300U／L 118例，异常检出率30．76％。明显高于血清Ca、P和ALP异常检出率4．8％。BALP活性增高程度与母孕期日晒少、饮食结构不合理，未服用维生素D制剂有明显相关性（P〈0．01）；与胎龄、体重有明显相关l生（P〈0．01）.结论 BALP的测定是筛查先天性佝偻病的优选方法。目前先天性佝偻病的发病率仍较高，预防先天性佝偻病应从围产期做起；对早产儿、低出生体重儿应及时检测BALP水平，以便早期诊断、早期治疗。

Analysis of the Determination of BALP in Early Neonates and the Related Factors of Congenital Rickets

Objective To evaluate the value of bone alkaline phosphatase in the screening of congenital rickets, and to assess the morbidity of the newborn congenital rickets and its relined factors. Methods BALP in 630 early neonates with dry chemical immunoinspissation of whole blood was determined, the relates of the active level of BALP and its influencing faotors was analyzed and contrasted with the level of calcius,phosphorus and alkaline phosphatase （ALP） of serum. Results Of the 630 cases,430 cases were BALP≤200U/L,82 cases were ≤250U/L,118 cases were≤300U/L,30.76% wer unnormal. It was much higher than that of calcium,phosphorus and alkaline phosphatase,and those were 4.8%.There Was a significantly positive correlation between the active of BALP with less sunshine in gestational period,mageiric formation is unscientific and do not peros pharmaceutics if Vitamin D （P〈 0.01）. There was positive correlation between the active of BALP with the age of gestation and birth weight（P〈 0.01）. Conclusion The determination of BALP is the optimizing method in screening of congenial rickets. For the present time,the morbidity is at the high level in the congenial rachitis. We must do begin with perinatal period to prevent the diseases; and we should test the level of BALP for the premature infants and those with substandard birth weight. So we could diagnose the disease early and treat it early.
