氟比洛芬酯用于冠状动脉搭桥术后镇痛的效果评价

目的比较冠状动脉搭桥术后患者自控静脉镇痛（PCIA）中氟比洛芬酯联合芬太尼与单纯芬太尼的镇痛效果与不良反应。方法选择冠状动脉搭桥术后行PCIA患者共40例,随机分为两组。A组：芬太尼10μg/kg＋托烷司琼5mg＋生理盐水至100ml;B组：氟比洛芬酯100mg＋芬太尼5μg/kg＋托烷司琼5mg＋生理盐水至100ml。PCIA泵的设置均为维持量2ml/h,单次负荷剂量0.5ml,锁定时间15min。观察两组术后24、48h的镇痛、镇静、舒适度评分和不良反应发生率。结果两组术后24、48h镇痛评分及舒适度评分均无显著性差异。A组术后24h镇静评分为3.59±0.48,B组为2.12±0.33,有显著性差异。B组（3例）不良反应发生率低于A组（19例）。结论氟比洛芬酯联合芬太尼用于冠状动脉搭桥术后PCIA的镇痛效果良好,能明显降低芬太尼的用量和不良反应发生率。

Clinical evaluation of analgesjc effect of flurbiprofen axetil in patients after coronary artery bypass grafting surgery

Objective To compare the clinical evaluation of analgesic effect and adverse effects of flurbiprofen axetil combined with fentanyl and fentanyl alone in patients controlled intravenous analgesia after coronary artery bypass grafting surgery. Methods Totally 40 patients, with controlled intravenous analgesia after coronary artery bypass grafting surgery, were randomly divided into two groups. Group A： fentanyl 10μg/kg＋tropisetron 5mg＋0.9N.S to 100ml. Group B： flurbiprofen axetil 100mg＋fentanyl 5μg/kg＋tropisetron 5mg＋0.9N.S to 100ml.PCA setting： background infusion 2 ml/h, Blous dose of 0. 5ml, lockout time of 15 min. The use of PCA pumps 24h,48h were evaluated by visual analgesia scales, ramsay scales and patient satisfied scales, the adverse effects were also recorded. Results Two groups had no difference in visual analgesia and satisfied scales. There was obvious difference in the 24h ramsay scales. The adverse effects of group B were less than that of group A. Conclusions Flurbiprofen axetil combined with fentanyl in patients controlled intravenous analgesia after coronary artery bypass grafting surgery has satisfactory analgesic effects. It can decrease the dosage of fentanyl and the rate of adverse effects.
