大枫子膏联合抗敏1号方治疗特应性皮炎30例临床观察

目的 评价大枫子膏联合中药汤剂治疗特应性皮炎的临床疗效.方法 将60例特应性皮炎患者随机分为治疗组和对照组各30例,两组均口服常用经验方“抗敏l号方”的基础上进行加减辨证用药,同时治疗组使用大枫子膏涂于患处皮肤,对照组使用他克莫司软膏涂于患处皮肤,两组均早晚各1次,每日用量不超过10g.4周为1个疗程,两组均治疗2个疗程.观察两组患者临床症状积分和主要单项症状情况,评价临床综合疗效.结果 治疗组总有效率93.33％,对照组总有效率96.67％,两组患者临床疗效比较差异无统计学意义(P＞0.05).对照组与治疗组在治疗前后SCORAD评分、皮损范围、皮损严重程度及瘙痒和睡眠影响程度各项评分差异均有统计学意义(P＜0.05)；两组第2个疗程与第1个疗程组内比较差异有统计学意义(P＜0.05).治疗后第1个疗程和第2个疗程组间比较,瘙痒和睡眠影响程度改善方面治疗组优于对照组(P＜0.05).治疗组皮肤干燥、表皮脱落改善程度优于对照组(P＜0.05),两组在丘疹水肿、渗出或结痂、苔藓化方面差异无统计学意义(P＞0.05).结论 大枫子膏联合抗敏1号方治疗特应性皮炎疗效确切,无明显不良反应.

Clinical Observation on Dafengzi Ointment Combined with Chinese Medication Decoction for 30 Cases of Atopic Dermatitis

Objective To evaluate the effect of Dafengzi OintmentOintment of hydnocarpus(Hydnocarpi Semen)] on atopic dermatitis(AD).Methods Sixty AD patients were randomized into treatment group and control group,with 30 in each.Both groups were given modified decoction based on Kangmin Yihao Formula(No.1 Formula for antianaphylaxis).Dafengzi Ointment and Tacrolimus Ointment was applied in the treatment group and control group respectively,twice daily,in the morning and evening.The daily dosages of both ointments were within 10g.Four 4 weeks as a treatment course,and the treatment lasted for 2 courses.The therapeutic effect was evaluated through observing the clinical symptom scores and the main single symptom.Results The total effective rate of the treatment and control group was 93.33% and 96.67% respectively.The difference in the clinical effect between groups was not statistically significant(P>0.05).Both groups were significantly effective in scoring atopic dermatitis(SCORAD),skin lesions,sleep quality and pruritus(P<0.05).Comparing with in-group,the differences between the first treatment course and second treatment course were statistically significant in both groups(P<0.05).Comparing with the same timing,there was no significant difference between groups in SCORAD and skin lesions(P>0.05).Comparing with the control group,the treatment group could significantly improve sleep quality,relieve pruritus,asteatosis and excoriation(P<0.05).There was no significant difference between groups in relieving papule,edema,exudation,incrustation and lichenification(P>0.05).Conclusion Dafengzi Ointment is effective for AD with fine clinical application.
