结节性甲状腺肿和甲状腺腺瘤囊变的超声鉴别诊断

目的；通过分析结节性甲状腺肿和甲状腺腺瘤发生囊性病变时的声像图特征及相应的病理学基础，探讨结节性甲状腺肿和甲状腺腺瘤的超声鉴别诊断要点。方法：对经手术病理证实的92例结节性甲状腺肿与44例甲状腺腺瘤的超声声像图表现作统计学分析。结果：两组病例中，性别，年龄，病灶最大直径，病灶液性与实性回声的比例，病灶周边有否晕环等指标，均无显著性差异（P＞0．05）；双侧叶肿大否，病灶是否单发，病灶有无包膜，囊腔内壁有否乳头状突起等指标，均有显著性差异（P＜0．05）。结论：结节性甲状腺肿和甲状腺腺瘤两者的病理基础不同，构成了超声图像上的特征，可作为两病的超声鉴别诊断要点。

Ultrasonographic Differential Diagnosis Between Nodular Goiter and Thyroid Adenoma with Cystic Lesions

Objective:To investigate ultranosographic characteristics of nodular goiter and thyroid adenoma with cystic lesions,to reveal pathologic background, and to look for their characteristics on ultrasonographic differential diagnosis.Methods:Ninety two cases with nodular goiter and 44 cases with thyroid cystic chnanged adenoma were confirmed by pathologic diagnosis and the relationship between the ultrasonographic characteristics and their pathologic features were analyzed.Results:The two groups showed no significant differences ( P >0 05) in sex,age, diameters of lesions,ratio of liquid focus to solid focus, and faint rings around the foci In foct there were significant differences ( P <0 05) in tumescence of bilateral thyroid gland,single or plural foci,focal capsules and papillary protrusions within cysts.Conclusions:Because the pathologic basis of nodular goiter and thyroid adenoma are different, they have their own ultrasonographic characteristics So we can make differential diagnosis between these two diseases by ultrasonography.
