腹腔镜下直肠癌全直肠系膜切除术治疗直肠癌的效果观察

目的：对比、分析腹腔镜下全直肠系膜切除术和开腹 TmE 术治疗直肠癌的临床疗效及并发症。方法回顾性分析2012年2月-2013年7月收治的81例行2种不同手术的直肠癌患者的病历资料，比较2组患者的临床疗效以及并发症发生率。结果腹腔镜组患者平均手术时间明显长于开腹组（P ﹤0.05）；但前者的术中出血量、术后镇痛时间、术后排气时间、住院时间以及留置导尿管时间均显著短于开腹组患者（P ﹤0.05）；2组患者在切除淋巴数、保肛率以及环切缘受累率方面比较差异无统计学意义（P ﹥0.05）。腹腔镜组患者术后并发症发生率低于开腹组（P ﹤0.05）。结论腹腔镜下行全直肠系膜切除术疗效好、并发症少，值得临床推广应用。

Effect observation of laparoscopic total mesorectal excision in the treatment of colorectal cancer

Objective To compare and analyze the clinical efficacy and complications of laparoscopic total mesorectal excision and open TmE in the treatment of colorectal cancer. Methods The medical records of 81 patients with colorectal cancer with two different surgeries in our department from February 2012 to July 2013 were taken for retrospective analysis. The clinical efficacy and complication rates of 2 groups were compared. Results The mean operative time of laparoscopic group was significantly longer than that of the laparotomy group（P ﹤ 0. 05）;but the blood loss,postoperative analgesia,postoperative discharge,hospitalization time and indwelling catheter time in the former were significantly shorter than the laparotomy group patients（P ﹤ 0. 05）;the two groups in the number of resected lymph,rectal resection margin involvement rate,and rate of ring were without significant differences（P ﹥ 0. 05）. Postoperative complications in the laparoscopic group was lower than that of the laparotomy group（P ﹤ 0. 05）. Conclusion The effect of laparoscopic total mesorectal excision is good,with fewer compli-cations,which is worthy of promotion.
