胸膜孤立性纤维瘤的胸腔镜手术治疗

目的:探讨胸腔镜手术(video-assisted thoracoscopy,VATS)治疗胸膜孤立性纤维瘤(solitary fibrous tumors of the pleura,SFTP)的可行性,总结微创治疗经验。方法:回顾性分析经VATS手术切除5例SFTP的临床资料。结果:手术时间75~140 min,平均115 min。术中完整切除肿瘤,无并发症及围手术期死亡。术中探查肿物包膜较完整、无外侵,术中证实3例肿瘤起源于脏层胸膜,2例起源于壁层胸膜。确诊依据病理和免疫组化分析,纳入患者Vimentin和CD34均阳性表达。术后随访5~36个月,无复发和转移。结论:经VATS切除SFTP具有创伤小、能完整切除肿瘤、安全可行、术后并发症少等优点,临床值得推荐。

Thoracoscopic treatment for solitary fibrous tumors of the pleura

Objective:To explore the feasibility of Video-Assisted Thoracic Surgery(VATS)in the treatment of solitary fibrous tumors of the pleura (SFTP)and to summarize our experience of minimally invasive treatment.Methods:We retrospectively analysis the clinical data of 5 patients with SFTP by VATS.Results:The operation time ranged from 75 to 140min,(mean 1 15 min).All tumor were completely excised.There was no complication and death of perioperative period.During the operation period,the capsule of goitre was explored to be complete and wasn’invaded.It was found that tumors originated from visceral pleura in 3 cases,parietal pleura in 2 cases.Confirmed by pathological and immunohistochemical analysis,Vimentin and CD34 were positive expression in patients.All of the patients were followed up for 5-36 months and no death,no recurrence and metastasis.Conclusion:VATS resection SFTP has the ad-vantages of small trauma,complete resection of tumor,safe and feasible and less postoperative complications,which is worthy to recom-mend in clinical.
