常见阴道炎病原体检测及微生态状况分析

摘    要:目的:了解本地区女性阴道炎患者病原体感染及微生态状况,为阴道炎的临床诊治提供依据。方法:采用阴道分泌物常规显微镜检联合需氧菌阴道炎(aerobic vaginitis,AV)/细菌性阴道病(bacterial vaginosis,BV)五项预成酶谱分析技术对21 812例妇科就诊患者的阴道分泌物进行检测。结果:(1)21 812例阴道分泌物送检标本检出假丝酵母菌阴道炎(vulvovaginal candidiasis,VVC)、BV、AV、滴虫性阴道炎(trichomonas vaginitis,TV)单一感染及各类混合感染共计13 510例,总检出率61.94%;13 510例感染标本中,单一感染6 391例占47.31%,混合感染7 119例占52.69%。混合感染中,AV合并VVC2 323例,占32.63%,构成比最高;其次为AV合并BV 2 113例占29.68%。(2)检测结果按4个月份段进行统计分析,VVC在1~3月检出844例(15.12%);混合感染在7~9月检出2 328例(42.48%),检出率均明显高于其他月份段,比较差异有统计学意义(P<0.05)。(3)21 812例阴道分泌物送检标本中,乳酸杆菌失衡检出率78.19%,常见阴道炎与阴道菌群失调密切相关,VVC、BV、AV、TV及混合感染菌群失调的检出百分比分别为83.85%、95.67%、100%、96.97%、100%。结论:本地区常见阴道炎及混合感染发生率高,阴道菌群失调多见,混合性阴道炎以AV混合感染为主,VVC和混合性阴道炎的感染与季节相关。

Abstract:Objective:To understand the infections of pathogens and micro-ecological status of local female patients with vagini-tis,and provide evidence for clinical diagnosis and treatment of vaginitis.Methods:By using routine vaginal microscopy combined with aerobic vaginitis (AV)/bacterial vaginosis (BV)five pre enzyme spectrum analysis of vaginal secretion in 21812 cases of gynecolog-ical patients were detected.Results:There were altogether 13 510 vaginal secretion specimens that had been detected with vulvovaginal candidiasis(VVC),bacterial vaginosis(BV),aerobic vaginitis(AV)and trichomonas vaginitis(TV)and the total detection rate was 61.94%.Among the 13 510 specimens,6 391 specimens(47.31%)were single infection and 7 119 specimens(52.69%)were mixed infection.Among the mixed infection,there were 2323 cases of AV complicated with VVC,which accounted for 32.63%,2113 cases of AV complicated with BV which accounted for 29.68%.The detected results were divided into four segments of month for statistical a-nalysis.844 cases of VVC (15.12%)were detected 1-3 months and 2328 cases of mixed infections (42.48%)were detected 7-9 months.The detection rates were significantly higher than in other segments of month and the differences were statistically significant (P<0.05).The detection rate of lactobacilli dysbacteriosis was 78.19%.Vaginal dysbacteriosis and common vaginitis was closely relat-ed.The lactobacilli dysbacteriosis rate of VVC,BV,AV,TV and mixed infection was respectively 83.85%,95.67%,100%,96.97%and 100%.Conclusion:There is a high incidence of common vaginitis and mixed infection in local region.Common vaginitis and mixed infection rate in the region is high,vaginal flora imbalance,mixed vaginitis mixed with AV infection,VVC and mixed vaginitis infection and seasonal correlation.
