双侧额叶重度脑挫裂伤伴发中央型脑疝的诊治

目的探讨双侧额叶重度脑挫裂伤伴发中央型脑疝的临床特征及手术方法。方法对35例手术救治的双侧额叶重度脑挫裂伤并发中央型脑疝的临床资料进行分析。结果35例中良好17例、轻残7例、重残2例、植物状态1例、死亡2例。结论熟悉中央型脑疝前期或间脑期临床表现，重视影像学特征性表现，迅速正确诊断，尽早开颅减压，能提高患者治愈率，改善预后。

Diagnosis and treatment of severe bilateral frontal brain contusion complicated central hernia

Objective To explore clinical characteristics and surgical methods of severe brain contusion in bilateral frontal lobes complicated with central herniation.Methods The clinical data of 35 cases who had undergone surgical treatment with severe bilateral frontal brain contusion complicated by central herniation were analyzed.Results Among the 35 cases,results were different.17 cases had satisfied recovery,7 cases had mild disability,2 cases had severe disability,1 case survived without any consciousness,and 2 cases were dead.Conclusions It is very important to be familiar with the early clinical manifestations of central herniation or brainstem period.Moreover,we should pay attention to the imaging features in order to make right diagnosis promptly and perform cranioectomy for decompression as soon as possible to increase the successful treatment rate and improve the prognosis of the patients.
