| 摘 要： | 目的:探讨乳突轮廓化联合鼓膜置管治疗难治性分泌性中耳炎的临床疗效及机制.方法:回顾性分析2001-03-2010-03期间收治的难治性分泌性中耳炎患者22例(33耳),所有患者先后行单纯鼓膜置管3次或3次以上,但症状无明显缓解.采用乳突轮廓化联合鼓膜置管治疗,鼓膜置管术后3～6个月拔管.结果:22例患者术后自觉患侧听力恢复正常或有不同程度提高,耳内闷胀感消失;鼓膜呈灰白色,鼓膜动度无明显受限;纯音听阈检测骨气导差为(13.54±4.86)dB;术后29耳鼓室导抗图为A型,4耳为C型,30耳出现同侧声反射.结论:对于反复鼓膜置管3次或3次以上,病期迁延超过2年的难治性分泌性中耳炎患者,采用乳突轮廓化联合鼓膜置管治疗,在清除中耳鼓室、乳突病变的基础上,扩大了中耳、乳突气房容积,既改善了中耳腔的持续负压状态,又建立了乳突、鼓室至鼓膜置管的通畅引流,临床疗效满意.
