腹腔镜下高位精索血管结扎术治疗小儿精索静脉曲张疗效观察

观察腹腔镜下高位精索血管结扎（Palomo）术治疗小儿精索静脉曲张的临床疗效。方法：采用腹腔镜下Palomo术治疗小儿精索静脉曲张患者26例，患儿就诊年龄为11～15岁，平均12．5岁。均为左侧Ⅲ度精索静脉曲张。结果：手术时间30～90min，平均45min，平均术后住院3天。平均随访时间为15个月（3～41个月），均无术侧精索肿痛、附睾炎、睾丸萎缩及精索静脉曲张复发。结论：腹腔镜损伤小，且通过光学放大作用能提供快速的手术径路，因此，采用腹腔镜下Palomo手术治疗精索静脉曲张是一种安全有效的方法。

Laparoscopic Palomo Varicoelectomy in Children

Objective:To evaluate our experience using laparoscopic Palomo varicocele high ligation in children. Methods:Between 2006 and 2010,26 boys with primary varicocele were evaluated retrospectively.Outcome variables recorded for analysis were age at symptoms,varicocele grade(DubineAmelar classification),testicular atrophy, and length of hospital stay,perioperative complications,and recurrence after surgery.Mean follow up was 15 months(3-41 months).Results:Age at surgery ranged between 11 and 15 years(median 12.5 years).All patients had gradeⅢvaricocele at left side,and they were treated with laparoscopic Palomo varicocelectomy.Mean operative time was 45 min(30 to 90 min).Median hospital stay was 3 days,and none of them has testicular atrophy on postoperative followup.Recurrent varicoele was not observed with a success rate of 100%.Conclusions:Laparoscopic Palomo varicocelectomy is safe and effective for treatment of varicocele in children.
