26例胰岛素瘤的诊断及外科治疗分析

目的分析胰岛素瘤的临床表现特点,探讨胰岛素瘤的诊断及外科治疗方法。方法对该院自2001年12月至2009年6月收治的26例胰岛素瘤患者的临床资料进行回顾性分析。并结合国内外有关文献报道进行诊治分析。结果本组中25例有典型的Whipple三联征。术前以B超、选择性动脉造影及多层螺旋CT进行诊断和定位。26例均行手术治疗,均经病理证实,其中良性胰岛素瘤23例,恶性胰岛素瘤3例。术前B超、选择性动脉造影及CT诊断的阳性率分别为46.2%(12/26),61.5%(8/13)和69.6%(16/23),术中24例进行B超检查,22例与术中探查结果相符。行单纯肿瘤摘除术19例,局部切除术4例,胰体尾部切除术2例,胰十二指肠切除术1例。术后血糖均恢复正常水平。有1例发生胰瘘并发症,1例发生假性囊肿。结论胰岛素瘤临床主要表现为典型的Whipple三联征。术前以B超,多层螺旋CT检查及选择性动脉造影明确定位。对良性胰岛素肿瘤宜行单纯肿瘤切除或胰腺部分切除,对恶性肿瘤应采用扩大范围的根治性肿瘤切除术为主的综合治疗。

Analysis of diagnosis and surgical treatment of insulinoma in 26 cases
