对高龄初产妇11年的回顾性分析

目的 了解近11年高龄初产发生率、妊娠并发症、合并症及其妊娠结局的变化,以加强对高龄初产的围产管理。方法 对1989年-1999年在我院分娩的高龄初产妇186例进行回顾性分析,并与同期正常年龄组初产妇对照。结果 近11年高龄初产发生率明显增高(P<0.01),既往有不良孕史、并发妊高征、合并子宫肌瘤者较对照组增多(P<0.05),但并未逐年增多。剖宫产率较对照组逐年增多(P<0.001),单纯以高龄初产为手术指征者明显增多(P<0.01)。新生儿体重明显逐年增加(P<0.01)。结论 社会因素造成高龄初产逐年增多,因围产保健的加强,并发症、不良妊娠结局并未逐年增多,不提倡以高龄初产为单一因素选择剖立产。

changes of the elderly primiparas in the past eleven years

Objective To describe the changing of perinatal risk factors of elderly primipara (age>35 years) in order to strengthen systematic management of their perinatal care. Methods The clinical datas of 186 cases of elder primiparas delivered in our hospital from Jan 1989 to Dec 1999 were analysed retrospectively. The younger primiparas delivered at the same time were used as the control group. Results Incidence of elder primiparas was significantly increased in the past eleven years ( P < 0.01). There were more complications of pregnancy or delivery among elder primiparas than the younger primiparas(P < 0.05). The indication of cesarean section only for the age was higher in the elder mothers than that of control group ( P < 0.001). The neonatal birth weight was increased gradually ( P < 0.01). Conclusions Social trends have encouraged women to delay child-bearing. Improvement of perinatal care provides a good outcome of elder primiparas, so that only age of the primiparous will not be the indication for cesarean section.
