摘    要:目的 探讨经鼻胃管(NGT)与经皮内镜下胃造瘘肠内营养在鼻咽癌患者中的临床应用.方法 研究2010年5月-2013年6月我院收治的鼻咽癌患者50例,分为NGT组25例经鼻胃管肠内营养,PEG组25例经皮内镜下胃造瘘肠内营养,比较分析治疗前后2周、治疗后4周及治疗后6周两组患者的体重指数(BMI)、血红蛋白(HGB)、白蛋白(ALB)、前白蛋白(PA)营养指标情况,同时比较两组治疗前后肺部感染、反流性食管炎、消化道出血并发症的发生情况.结果 PEG组患者营养指标高于NGT组(P<0.01),不同时间点之间比较,差异有统计学意义(P<0.01).治疗6周后PEG组患者肺部感染、反流性食管炎、消化道出血的发生率低于NGT组(P<0.01).结论 PEG的肠内营养可改善鼻咽癌患者的营养不良,能降低并发症的发生率,值得临床推广.

Application of percutaneous endoscopic gastrostomy enteral nutrition in patients with nasopharyngeal carcinoma

Abstract:Objective To evaluate the efficacy and safety of nasogastric tube (NGT) and percutaneous endoscopic gastrostomy (PEG) for patients with nasopharyngeal carcinoma. Methods Fifty patients with nasopharyngeal carcinoma admitted to our hospital from May 2010 to June 2013 were selected. The patients were divided into NGT group and PEG group, with each group twenty five cases. Body mass index (BMI), hemoglobin (HGB), albumin (ALB) and prealbumin (PA) were com- pared and analyzed before treatment, at the second week of the treatment, at the fourth week of the treatment and at the sixth week of the treatment. Meanwhile, pulmonary infection, reflux esophagitis and alimentary tract hemorrhage were also compared between the two groups before and after treatment. Results The nutrition indicators of PEG group patients was significantly higher than NGT group (P 〈 0. 01 ). The nutrition indicators of the two groups at different times also showed statistically signifi- cant difference (P 〈0. 01 ). After six weeks treatment, the incidence of pulmonary infection, reflux esophagitis and alimentary tract hemorrhage of PEG group signifieantly lower than the NGT group ( P 〈 0. 01 ). Conclusion PEG enteral nutrition can improve malnutrition in patients with nasopharyngeal carcinoma, can significantly reduce the incidence of complications, it is worth promoting.
