宫腔镜、腹腔镜下处理迷路IUD的体会

目的 探讨宫腔镜、腹腔镜下迷路宫内节育器取出术的方法。方法 对44例子宫内迷路IUD患者应用宫腔镜取出，术前宫颈预处理，术时根据类型不同采用不同的手术方法，部分患者为预防子宫穿孔行B超或腹腔镜监护。对5例异位于子宫外IUD患者应用腹腔镜取出。结果 49例迷路IUD均一次取器成功．无并发症发生。结论 对于迷路节育环的处理方案是不定型的。对于子宫内迷路IUD术前须充分扩张子宫颈，手术时视情况采用不同的设备和方法。腹腔镜处理异位于子宫外的IUD优于剖腹手术。

REALIZATION OF HYSTEROSCOPY OR LAPAROSCOPY DEALING WITH ECTOPIC IUD

Objective:To evaluate the method of taking out ectopic IUD with hysteroscopy or laparoscopy.Methods:Ectopic IUD inside uterine of 44 cases was taken out by hysteroscopy.Cervix was pre-treated before operation and different surgical operations were adopted in terms of different types of intra -operation. Parts of patients were guarded by laparoscopy or B ultrasonography to prevent uterine perforation from operation. Five cases of them were taken out by laparoscopy.Results:All of the 49 cases with ectopic IUD were successful in the first operation and had no complications occurrence.Conclusions:Management of ectopic IUD is uncertain the type. Pre-operation of ectopic IUD inside uterine must sufficiently expand cervix and adopt the different equipments and ways according to intra-operation situation.Laparoscopy handles ectopic IUD outside uterine better than gynecologic surgical procedures.
