二氧化碳气体温度对腹腔镜胃肠手术后患者恢复的影响

摘    要:【目的】探讨二氧化碳(CO2)气体温度对腹腔镜胃肠手术后患者恢复的影响。【方法】125例进行胃肠手术的患者随机分为两组。试验组62例,采用加温至37℃的 CO2气体建立气腹;对照组63例,采用常规方法建立气腹,观察两组患者手术前后的体温、气管插管拔管时间、肛门排气时间、胃管拔除时间,对手术伤口进行疼痛数字评分量表(NRS)评分。【结果】两组患者手术前体温比较无显著差异(P >0.05),术后试验组体温明显高于对照组(P<0.05)。试验组手术后气管插管拔除时间、肛门排气时间、胃管拔除时间和伤口疼痛评分均明显低于对照组,两组相比有显著差异(P <0.05)。试验组发生寒战者6例(9.7%),明显低于对照组的35例(55.6%),两组相比有显著差异(P<0.05)。试验组胃肠功能紊乱者7例(11.3%);明显低于对照组的26例(41.3%),两组相比有显著差异(P <0.05)。【结论】采用加温至37℃的 CO2气体建立气腹,可促进腹腔镜胃肠手术后患者的恢复。

Abstract:Objective]To explore the influence of the temperature of carbon dioxide(CO2 )gas on the re-covery of patients after laparoscopic gastrointestinal operation.Methods]Totally 125 patients after laparo-scopic gastrointestinal operation were divided into 2 groups.In the experimental group(n=62),pneumoper-itoneum was established with CO2 gas heated to 37℃.In the control group(n=63),pneumoperitoneum was established with routine method.Body temperature before and after operation,tracheal extubation time,anal exhaust time and gastric extubation time in two groups were observed.Numerical rating scale(NRS)was used to evaluate the pain of surgical wound.Results]There was no significant difference in body temperature be-fore operation between two groups (P〉0.05).After operation,body temperature in the experimental group was obviously higher than in the control group(P〈0.05).The tracheal extubation time,anal exhaust time, gastric extubation time and wound pain score in the experimental group were obviously lower than those in the control group,and there was significant difference(P〈0.05).The incidence of postoperative complications was compared between two groups.Chills occurred in 6 cases(9.7%)of experimental group,which were sig-nificantly lower than those of control group(35 cases,55.6%),and there was significant difference between two groups(P〈0.05).Gastrointestinal dysfunction occurred in 7 cases(11.3%)of experimental group, which were significantly lower than those of control group(26 cases,41.3%),and there was significant differ-ence between two groups(P〈0.05).Conclusion]Pneumoperitoneum established by CO2 gas heated to 37℃can promote the recovery of patients after laparoscopic gastrointestinal surgery.
