高分辨率CT扫描对肺内孤立性占位的诊断价值

目的 比较高分辨率CT扫描(HRCT)和常规CT扫描(CCT)在诊断肺内孤立性占位方面的价值。方法 回顾性分析48例直径为0.8-6.0cm先后进行了CCT和HRCT扫描检查,且经病理或临床证实诊断的肺内孤立性占位。结果 1)HRCT较CCT更能清晰地显示肺内的细微结构;2)HHCT在显示周围型肺癌的形态学表现如:胸膜凹陷征,血管纠集征,支气管引入征,毛刺,空泡征与CCT有显著的统计学差异(p<0.01);在发现分叶,空洞,钙化方面无明显差异。3)HRCT对恶性肿块周围的血管浸润分辨更清晰。4)HRCT对结核球的卫星病灶,粗长毛刺,钙化特点显示较CCT清晰。5)HRCT能准确清晰地显示占位同支气管的关系,利于指导临床的纤维支气管镜检查。结论 高分辨率CT在肺内孤立性占位诊断的准确率方面较常规CT高。

Diagnostic value of HRCT in SPN

Purpose To compare the capability of high resolution CT (HRCT) with conventional CT (CCT) for evaluating solitary pulmonary nodules. Methods Forty - eight patients of SPN between 0. 8cm and 6. Ocm in diameter confirmed by pathology were examined with HRCT and CCT before surgery and analyzed retrospectively after postoperation. Results (1) Tiny anatomical structure is shown more clearly by HRCT than CCT. (2) There were marked statistics difference (p<0.01) in finding small lung cancer between HRCT and CCT. (3) Bundle vessels surrounding lung cancers can be shown in HRCT. (4) Satellite lesions, thick and long spicule and calcification in Tuberculosis ball could be displayed by HRCT better. (5) HRCT was useful not only for depicting the presence of the positive bronchus sign but also relationship of tumor- bronchus. Conclusions HRCT is more useful for accurate diagnosis of solitary lung nodules than CCT.
