螺旋CT在胆囊癌诊断中的价值

目的探讨螺旋CT对胆囊癌的诊断价值。方法回顾性分析48例经手术或病理证实的胆囊癌患者的螺旋CT资料,所有病例均经螺旋CT平扫及多期动态增强扫描,并进行MPR、MIP及CPR等后处理。结果胆囊癌CT直接征象表现为：胆囊壁肥厚型11例、结节型14例、肿块型23例,病灶均明显强化,伴有胆囊黏膜破坏、囊壁僵硬;间接征象有：合并胆囊结石26例、邻近侵犯17例、肝脏转移12例、淋巴结转移15例、胆道梗阻13例。39例术前诊断与手术病理及随访结果一致。结论螺旋CT增强扫描结合多种方法重组对早期发现胆囊癌具有重要的诊断价值,为临床治疗和手术方案提供可靠依据。

Value of spira CT in diagnosing gallbladder carcinoma

Objective To study the helical CT diagnostic value of galbladder carcinoma. Methods Helical CT findings of 48 patients with gallbladder carcinoma confirmed surgically and pathologically were analyzed retrospectively. All cases were examined by CT plain scan and muetiphase dynamic enhanced scan, and multi-planar reformation （MPR）, maximum intensity projection （MIP） and curved planner reconstruction（CPR） were adopted. Results Direct signs of gallbladder carcinoma on helical CT included the type of thick wall in 11 cases, the type of intraluminal nodules in 14 cases, the type of mass in 23 cases, lesion with marked enhancement and destruction of gallbladder mueosa. Indirect signs included con- comitant gallbladder stones （26）, neighbodng violations （17）, liver metastasis （12）, lymph node metastasis （15）, and bil- iary obstruction （13）. The preoperative imaging diagnosis was consistent with surgical pathology and follow-up results in 39 cases. Conclusion The enhanced scan and multi-image reconstruction of helical CT have an important diagnostic value for the early detection of gallbladder carcinoma, providing reliable basis for clinical treatment and srgery.
