儿童围术期焦虑及心理干预的探讨

目的 观察心理干预对儿童围术期焦虑的影响.方法 择期手术患儿60例.ASA Ⅰ～Ⅱ级,分为1～3岁、3～7岁、7～12岁3个年龄段,每年龄段均为20例,并根据不同干预方法 分为诱导期家长陪同组(PP组)和术前参观手术室组(V组),每组10例.采用改良耶鲁儿童术前焦虑测评量表(mYPAS)观察术前访视(T1)、人手术室时(T2)、诱导时(T3)、术后1d(T4)4个时间点的焦虑水平.以诱导期合作评分(ICC)观察诱导合作程度.结果 1～3岁患儿中P组焦虑评分在人室和诱导期明显低于V组,诱导期合作程度也明显好于V组:3～7岁患儿中V组入室和诱导期的焦虑评分及诱导期合作程度明显好于P组:7～12岁患儿中P组和V组都表现了较低的焦虑和较好的合作.结论 对儿童进行围术期心理干预应根据其年龄特点而有所不同.对于年龄较小的患儿采用家长陪同的方法 效果较好,对于年龄较大的患儿宜采用参观手术室或两者结合的方法 .

Perioperative anxiety in children and the use of psychological intervention
