清瘟败毒饮治疗脓毒症疗效观察及对细胞因子的影响

目的观察中药复方清瘟败毒饮对脓毒症的，临床疗效以及对血清促炎因子肿瘤坏死因子Ⅸ（TNF—α）、白细胞介素（IL）-6、IL-1和抗炎因子IL-10水平的影响。方法将38例脓毒症患者随机分为治疗组（西医常规治疗加清瘟败毒饮）18例和对照组（西医常规治疗）20例，疗程7d，分别比较治疗前后中医症状积分、APACHEII评分以及外周静脉血TNF—α、IL-1、IL-6和IL-10水平的变化。结果2组总疗效比较差异无统计学意义（P〉0．05），但治疗组临床控制率优于对照组（P〈0．05）；治疗后2组中医症状积分和APACHEII评分分值均明显下降（P〈0．05），治疗组优于对照组（P〈0．05）；治疗后2组患者的TNF—α、IL-1、IL-6和IL-10水平均降低（P〈0．05，P〈0．01），治疗组优于对照组（P〈0．05）。结论清瘟败毒饮能提高脓毒症患者的临床疗效，可调节TNF—α、IL-6、IL-1和IL-10水平，抑制过度炎症反应。

Clinical Efficacy of Qingwen Baidu Drink in Treating Sepsis and Its Effect on Cytokine

Objective To observe the clinical efficacy of Qingwen Baidu drink in the treatment of sepsis, and effect on serum pro-inflammatory factor TNF-α, IL-6, IL-1 and anti-inflammatory factor IL-10 levels. Methods Thirty-eight cases of sepsis patients were randomly divided into treatment group （conventional western medical treatment plus QingwenBaidu drink treatment, 18 cases） and the control group （conventional western medicine treatment, 20 cases） with the course of 7 days. TCM symptoms score, APACHE II score, as well as peripheral blood TNF-α, IL-1, IL-6 and IL-10 levels in the two groups were compared before and after treatment. Results The overall therapeutic effect of the two groups showed no differences （P〉0.05）, while clinical control rate of the treatment group was better than the control group {P〈0.05）. After treatment, TCM symptoms score and APACHE II score decreased in both groups （P〈0.05）, and the treatment group was better than the control group （P〈0.05）. Serum level of TNF-α, IL-1, IL-6 and IL-10 decreased in the two groups （P〈0.05, P〈0.01）, and the treatment group was better than the control group （P〈0.05）. Conclusion Qingwen Baidu drink can improve the clinical efficacy of sepsis, regulate TNF-α, IL-6, IL-1 and IL-10 levels and inhibit excessive inflammatory response.
