经阴道超声和腹部超声对子宫内膜病变诊断价值的比较

目的:评价经阴道超声和腹部超声对子宫内膜病变诊断价值。方法:随机选取2007年09月—2009年12月间我院非妊娠妇女经手术或诊断刮宫后子宫内膜活检者158例,分别对其行阴道超声检查及腹部超声检查,判断其形态、位置、回声及其与肌层的关系,并统计两者对子宫内膜病变的临床诊断率。结果:158例子宫内膜疾病患者经阴道超声检查,发现子宫内膜息肉51例,子宫黏膜下肌瘤17例,子宫内膜增厚52例,子宫内膜癌1例,异常回声36例,临床诊断符合率为99.40%;经腹部超声检查,发现子宫内膜息肉42例,子宫黏膜下肌瘤9例,子宫内膜增厚51例,子宫内膜癌1例,异常回声31例,临床诊断符合率为84.80%。经卡方检验,两组间临床诊断符合率比较,差异有统计学意义(P<0.01)。结论:与腹部超声相比,经阴道超声对子宫内膜病变的诊断有较高准确性,不仅能发现微小病灶,还能准确检查出肌层的浸润情况,能为宫内膜病变的鉴别诊断提供有价值的信息。

Transvaginal Ultrasound and Abdominal Ultrasound Diagnosis of Endometrial Lesions Compared

Objective：To evaluate transvaginal ultrasound and abdominal ultrasound diagnosis of endometrial lesions.Methods：A randomly selected in September 2007 一 2009,12 non-pregnant women in our hospital by procedure or diagnosis after curettage were 158 cases of endometrial biopsy,transvaginal ultrasound,respectively,and their abdominal ultrasound to determine the shape and location,Echo and its relationship with the muscle,and statistics on both the clinical diagnosis of endometrial lesions rate.Results：158 patients with endometrial disease by transvaginal ultrasonography,51 cases of endometrial polyps,submucous myoma in 17 cases,52 cases of endometrial thickening,endometrial cancer,1 case of 36 cases of abnormal echo,clinical diagnosis rate was 99.4%.Abdominal ultrasonography,42 cases of endometrial polyps,submucous myoma,9 cases of endometrial thickening in 51 cases,1 case of endometrial cancer,abnormal echo in 31 cases,clinical diagnosis rate was 84.8%.The chi-square test between the two groups the rate of clinical diagnosis,the difference was significant,with statistical significance（P0.01）.Conclusion：Compared with abdominal ultrasound,transvaginal ultrasound in the diagnosis of endometrial lesions have a higher accuracy,not only to find small lesions,can accurately check the infiltration of the muscle,for the identification of endometrial lesions provide valuable diagnostic information.
