骨梗死的影像学诊断

摘    要:目的探讨骨梗死的影像学特征。方法对13例经临床及病理证实的骨梗死患者行X线、CT及MRI检查,分析其影像学表现,并将骨梗死分为急性期、亚急性期和慢性期,总结其影像学特征。结果骨梗死急性期X线、CT表现为干骺端骨质疏松,MRT1WI骨梗死灶中央部分呈等至高信号,T2WI呈高信号,T1WI骨梗死灶边缘呈环形低信号,T2WI呈环形高信号;亚急性期X线、CT表现为小的虫噬样改变,伴有斑点状钙化,MRT1WI骨梗死灶呈等或低信号,T2WI呈等或稍高信号,T1WI骨梗死灶边缘呈环形低信号,T2WI呈环形高信号;慢性期X线、CT表现为不规则状、蜿蜒状骨质硬化,MRT1WI和T2WI均呈低信号。结论MRI是诊断骨梗死急性期的最有效方法,在急性和亚急性期,MRI优于X线和CT,在慢性期,X线、CT和MRI均具有特征性表现。

The imaging diagnosis of bone infarction

Abstract:Objective To investigate the radiological feature of infarct of bone. Methods 13 cases of clinically and pathology confirmed underwent X-ray, CT and MRI scaning. The imaging feature were Analyzed and summarized, infarct of bone separate acute stage, subacute stage and chronic stage. Results Metaphysis osteoporosis was observed on X-ray and CT in acute stage, the midsection of infarct of bone was isointensity on T1WI and iso/hyperintensity signal on T2WI, the edge of infarct of bone was ring-shaped hypointensity on T1WI and hyperintensity signal on T2WI. Small worm-eaten change and stippled calcification was observed on X-ray and CT in subacute stage, the infarct of bone was iso/hypointensity on T1WI and iso/hyperintensity signal on T2WI, the edge of infarct of bone was ring-shaped hypointensity on T1WI and hyperintensity signal on T2WI. Irregular and wriggle osteosclerosis was observed on X-ray and CT in chronic stage, was hypointensity on T1WI and T2WI. Conclusion MRI is the most modus operandi in diagnose of infarct of bone on acute stage, MRI surpass X-ray and CT on subacute stage, the finding of X-ray, CT and MRI is specific on chronic stage.
