临床路径在小儿外科手术中应用的效果评价

目的:评价临床路径在小儿外科手术中应用的效果。方法:对我科2011年2-8月入院的腹股沟斜疝、鞘膜积液、包皮过长/包茎3个病种共284例患儿实行临床路径管理(CP组),选择前一年同期(2010年2-8月)同类疾病患儿共271例,实施传统医疗护理(非CP组)作为对照组。对2组的住院时间、住院费用、药品费用、抗菌药物费用、检查检验费用、护理费用、切口感染率及患者满意度等指标进行统计、分析。结果:CP组住院时间、住院费用、药品费用、抗菌药物费用、护理费用及检查检验费用均明显低于非CP组,患者满意度与非CP组比较明显提高,差异均有统计学意义(P<0.05)。2组术后切口感染率无明显差异(P>0.05)。结论:临床路径能明显缩短小儿外科住院患儿的住院时间,减少住院费用,规范医疗行为,提高患者满意度。

Effect Evaluation of Application of Clinical Pathway in Pediatric Surgery

OBJECTIVE： To study the implementation of the clinical pathway （CP） in the practice of pediatric surgery. METHODS： During Feb.-Aug. 2011, a total of 284 cases ofindirect inguinal henia, hydrocele and phimosis were accounted in clinical pathway management （CP group）. A total of 271 cases with the traditional care from Feb. to Aug. in 2010 constituted the control group （non-CP group）. The data of hospitalization days, hospital charges, medication costs, antibiotic costs, examination charges, nursing costs, incision infection rates and satisfaction degree of patients in 2 groups were analyzed statistically. RESULTS： Compared with non-CP group, CP group had shorter hospitalization stay, lower hospital charges, lower medication costs, lower antibiotic costs, lower nursing costs, lower examination charges, higher satisfaction degree of patients, there was statistical significance in difference （P0.05）. Incision infection rates had no statistically significant differences （P0.05）. CONCLUSION： The clinical pathway can reduce hospitalization days significantly, reduce hospital expenses, standardize medical behavior and improve satisfaction degree of patients.
