580例老年人动态心电图临床分析

目的 :探讨老年人心律失常和心肌缺血的发病率、发生机制及发生规律。方法 :对 135 3例进行 2 4h动态心电图 (DCG)监测的患者 ,做一回顾性的分析 ,按年龄分为老年组、中年组、青年组 ,分析每组各种心律失常、心肌缺血发生率、其他疾病的发生率及心律失常、心肌缺血与疾病和年龄的关系。结果 :老年组 (5 80例 )房性和室性心律失常均高于中、青年组 ,尤其是复杂性心律失常 (P <0 .0 1)。老年组冠心病、心肌缺血、窦性心动过缓、窦房阻滞或窦性停搏及束支传导阻滞等均高于中、青年组 (P <0 .0 1)。结论 :心律失常、心肌缺血的发病率随着年龄增加而增加 ,冠心病、高血压病随着增龄而增加 ,因此对于高血压病、冠心病老年患者应重视DCG检测 ,及时发现心律失常、心肌缺血并及时治疗。对于预防老年人恶性心律失常、心脏猝死的发生 ,有着重要的临床意义

A clinical analysis on 580 casesDCG of aged persons

Objective: To investigate the incidence of arrhythmia and myocardial ischemia and its rule in aged persons. Methods: Analysis of 1353 cases measured previously by Dynamic electrocardiogram (DCG) were divided into three groups: aged group?middle-aged groups and young group. analyze the incidence of various arrhythmias ?myocardial ischemia and other diseases in every group, and the relation between arrhythmia?myocardial ischemia and diseases and age . Results: Atrial arrhythmias and ventricular arrhythmia in aged group (580 cases) were all higher than that of middle and young group, especially complex arrhythmia (P<0.01). The coronary heart disease (CHD)? myocardial ischemia? sinus bradycardia?sino-atrial block or sinus pause and bundle branch block were all higher than that of middle-aged?young group (P<0.01 ). Conclusion: The incidence of arrhythmia, myocardial ischemia increase with increasing age, CHD?hypertension also increase with increasing age. hence, intensify DCG measure in aged patients with hypertension and CHD, early find arrhythmia and / or myocardial ischemia and treatment, prevent the occur of serious arrhythmia?sudden cardiac death, have important significance.
