| 摘 要： | 目的 探讨降低肝内胆结石术后高残石率的方法。方法 分析2年来采用切除术联合术中胆道镜治疗肝内胆结石58例，其中左外叶切除29例，左半肝切除12例，右后叶或右前叶局部肝切除17例。结果 术后残余结石8例（13.8%），其中左内叶2例，右后叶3例，右前叶3例，切口感染4例，胆漏2例，肝创面附近积液3例，其中1例发生反应性胸腔积液。结论 肝切除联合术中胆道镜是降低肝内胆结石术后残余率的有效方法。

| Abstract: | Objective To explore the effective method of avoiding high rate of the residual stones after operation of hepatolithiasis.Methods 58cases of hepatolithiasis were treated by hepatotomy combined with intraoperative choledochoscope over 2 years.29 cases had left lateral lobectomy,12cases had left hemihepatectomy,and 17 cases had sculptural hepatectomy.Results 8 cases had residual stones after operation,2 cases in the left medial lobe,3 cases in the right posterior lobe,and 3 cases in right anterior lobe.4 cases had incision infection,2 cases had bile leadage,3 cases had subphrenic effusion,and 1 case had thoracic effusion.Conclusion Treatment of hepatectomy combined with intraoperative choledochoscope is the effective method to treat hepatolithiasis. |
