MSCT和MRCP+常规MR在胆道梗阻中的应用对比研究

目的探讨MSCT及MRCP+MR对胆道梗阻的诊断价值。方法抽取2008～2009年50例胆道梗阻病例,均行MSCT平扫+增强扫描以及MRCP+MR,所有病例手术及病理证实。结果所有病例检查均获得成功,MSCT及MRCP+MR对胆道梗阻定位准确度均为100%,其中胆道结石准确度分别为88.5%(23/26)、92.3%(24/26),良性梗阻准确度分别为83.3%(10/12)、41.7%(5/12),恶性梗阻准确度分别为91.7%(11/12)、83.3%(10/12)。结论 MSCT及MRCP+MR均能对胆道梗阻正确定位,在胆道结石病变诊断中MRCP+MR稍高于MSCT(P0.05),在胆道梗阻良性病变定性诊断中MSCT较MRCP+MR准确性明显增高(P0.05),在恶性病变诊断中无明显差异(P0.05),因此,在对胆道梗阻病人的检查时MSCT较MRCP+MR更有优越。

The comparative study of MSCT and MRCP combined with MR in biliary tract obstruction disease

Objective To explore the diagnostic value of multi-slice spiral CT and MR cholangiopancreatography combined with MR for biliary tract obstruction diseases.Methods Fifty patients who were diagnosed as biliary tract obstruction between the year of 2008 and 2009 were took MSCT common scan,contrast lice scans,MRCP and MR.All patients were certified by the surgery and pathology results.Results All the cases were successfully checked,the accuracy of diagnosing biliary tract obstruction with MSCT and MRCP combined with MR were 100%,the qualitative accuracy were 88.5%（23/26） and 92.3%（24/26） respectively in biliary calculi group,83.3%（10/12） and 41.7%（5/12） respectively in benign obstruction group,while 91.7%（11/12） and 83.3%（10/12） in malignant obstruction group.Conclusion MSCT and MRCP combined with MR can accurately define the obstructive site in biliary tract obstruction.The MRCP combined with MR is more accurate than MSCT in biliary calculi group,and the MSCT is significantly more accurate than MRCP combined with MR in benign obstruction group,while there was no significant difference in malignant obstruction group.So MSCT examination is more advantageous than the MRCP combined with MR in biliary obstruction patients.
