465例剖宫产术预防用抗菌药物情况分析

目的:了解剖宫产手术围手术期预防用抗菌药物现状,为促进抗菌药物的合理使用提供参考。方法:回顾分析我院2012年6–12月465例剖宫产手术病例,对患者的一般情况、手术情况、抗菌药物使用时机、剂量、品种、用药时间以及用药频度(DDDs)和药物利用指数(DUI)等进行汇总分析。结果:465例剖宫产手术,预防用抗菌药物者464例(99.8%),其中,单用一种抗菌药物者391例(84.3%),联合用药者73例(15.8%);383例(82.5%)在断脐后给药;用药时间≥72 h、≤48 h、≤24 h者分别为314例(67.7%)、10例(2.2%)、140例(30.1%);用药频度居前三位的药物分别为头孢呋辛钠、头孢替安钠、奥硝唑。结论:我院剖宫产手术抗菌药物预防性用药时机、品种选择、联合用药等比较合理,但存在抗菌药物使用疗程过长、药物利用指数偏高的问题。

Analysis of the prophylactic antibacterial agents used in 465 patients with cesarean section

Objective： To investigate and analyze the application of prophylactic antibacterial agents during perioperative period in cesarean section and provide reference for rational drug use. Methods： A total of 465 cases of cesarean section collected from June to December 2012 were retrospectively analyzed in respect of the patient＇s general condition, surgical cases, medication time, dose, species, medication duration of antibacterial agents, DDDs and drug use index （DUI）. Results： A total of 464 （99.8%） patients were given prophylactic antibacterial agents, 391 （84.3%） patients with one antibacterial agents and 73 （15.8%） patients with the combination of antibacterial agents. A total of 383 cases （82.5%） were given antibacterial agents after cutting the umbilical cord. There were 314 cases （67.7%）, 10 cases （2.2%）, 140 cases （30.1%） in medication time ≥ 72 h, ≤ 48 h, ≤ 24 h respectively. The top three drugs in terms of DDDs were eefuroxime sodium, cefotiam and omidazole. Conclusion： The use of antibacterial agents in perioperative patients with cesarean section was basically rational in medication time, species choice, combination drug use, but the medication duration and DIM should be reduced further.
