胰岛素强化治疗对新诊断2型糖尿病胰岛功能的影响

目的：评价胰岛素强化治疗对新诊断2型糖尿病患者胰岛功能的影响;方法：96例新诊断2型糖尿病患者给予胰岛素多次皮下注射,应用预混胰岛素每日3次或长效胰岛素注射液与短效胰岛素注射液联合每日4次治疗,根据病情特点可联合口服药物双胍类及糖苷酶类,噻唑酮类药物治疗3月,前后检测血糖及胰岛素水平（行75g葡萄糖耐量试验监测5段血糖及胰岛素水平）,糖化血红蛋白,体重,前后对照研究.结果：96例患者胰岛素强化治疗后血糖明显下降,糖化血红蛋白明显下降,β细胞指数明显改善,胰岛素早相分泌指数I/G明显改善,胰岛素抵抗指数IR无明显统计学意义,体重无明显变化.结论：胰岛素强化治疗新诊断2型糖尿病患者,可积极控制血糖,明显改善β细胞功能及胰岛素早相分泌,而对体重及胰岛素抵抗改善不明显.

Effect of intensive insulin therapy on pancreatic islet function in newly diagnosed type 2 diabetes

objective： to evaluate the insulin intensive treatment on islet function in patients with newly diagnosed type 2 diabetes; Methods： 96 patients with newly diagnosed type 2 diabetes were given multiple subcutaneous insulin injection, the application of premixed insulin 2 to 3 times daily or long-acting insulin injection associated with short-acting insulin injection treatment, four times a day, according to patients condition can be combined oral drug mefformin and glycosidic enzyme, medication march thiazole ketones, testing blood glucose and insulin levels before and after the glucose tolerance test （line 75 g monitoring five period of blood glucose and insulin levels）, glycosylated hemoglobin, weight, before-and-after study. Results： 96 cases of patients after insulin intensive treatment significantly lower blood glucose, glycosylated hemoglobin decreased obviously, beta cells index improved significandy, early phase insulin secretion index bring I/bring G, insulin resistance index （IR has no obvious statistical significance, no obvious change in body weight. Conclusion： insulin intensive treatment patients with newly diagnosed type 2 diabetes, can actively control blood sugar, improve ss-cell function and early phase insulin ~secretion, and on body weight and insulin resistance improvement is not obvious.
