盖髓剂在外伤性恒前牙冠折露髓中的治疗效果

摘    要:目的 探讨外伤性恒前牙冠折露髓患者活髓切断术中不同盖髓剂对治疗疗效及钙化桥形成率的影响。方法 选取2019年5月至2021年5月温州市中西医结合医院收治的外伤性恒前牙冠折露髓患者104例(126颗牙),根据患者使用盖髓剂的不同将其分为氢氧化钙(calcium hydroxide,CH)组(34例,42颗)、矿物三氧化聚合体(mineral trioxide aggregat,MTA)组(33例,39颗)和iRoot BP Plus组(37例,45颗),比较三组患者的治疗疗效、牙根吸收情况、钙化桥形成率。结果 术后6个月,MTA组患者的有效率显著高于CH组(P<0.05),术后12个月,MTA组和iRoot BP Plus组患者的有效率均显著高于CH组(P<0.05);术后6个月、12个月,MTA组和iRoot BP Plus组患者的牙根吸收情况均显著优于CH组(P<0.05);术后3个月,MTA组和iRoot BP Plus组患者的牙钙化桥形成率显著高于CH组(P<0.05)。结论 外伤性恒前牙冠折露髓患者行活髓切断术时使用MTA和iRoot BP Plus的疗效及钙化桥形成率相当,均优于CH。

Effect of pulp capping agents in patients with traumatic crown fracture of permanent anterior teeth and exposed pulp
Abstract:Objective To investigate the influence of different pulp capping agents used in pulpotomy on effects and calcified bridge formation rate in patients with traumatic crown fracture of permanent anterior teeth and exposed pulp. Methods A total of 104 patients (126 teeth) with traumatic crown fracture of permanent anterior teeth and exposed pulp admitted to Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from May 2019 to May 2021 were selected. Patients were divided into calcium hydroxide (CH) group (34 cases, 42 teeth), mineral trioxide aggregat (MTA) group (33 cases, 39 teeth) and iRoot BP Plus group (37 cases, 45 teeth) according to the use of pulp capping agents. The curative effect, tooth root resorption and calcified bridge formation rate were compared among three groups. Results At 6 months after operation, the effective rate of MTA group was significantly higher than that of CH group (P<0.05). At 12 months after operation, the effective rate of MTA group and iRoot BP Plus group were significantly higher than those of CH group (P<0.05). At 6 months and 12 months after operation, tooth root resorption in MTA group and iRoot BP Plus group were significantly better than those in CH group (P<0.05). At 3 months after operation, calcified bridge formation rate in MTA group and iRoot BP Plus group were significantly higher than those in CH group (P<0.05). Conclusion Using MTA and iRoot BP Plus in pulpotomy of patients with traumatic crown fracture of permanent anterior teeth and exposed pulp can achieve comparable curative effect and calcified bridge formation rate, and both were better than CH.
